摘要
为探讨高频超声在儿童肠息肉合并肠套叠诊断中的应用价值,回顾分析2018年1月至2019年12月我院收治的65例肠息肉合并肠套叠患儿资料,患儿入院后均行腹部高频彩色多普勒超声检查,分析高频超声对肠息肉合并肠套叠的检出率,以及超声图像特征。结果显示,高频超声诊断肠息肉合并肠套叠59例(90.77%)。手术及肠镜检查证实单发息肉56例(息肉56枚),多发息肉9例(息肉20枚)。高频超声诊断肠息肉59例(息肉68枚),其中单发息肉52例(息肉52枚),多发息肉7例(息肉16枚),息肉检出率为89.47%(68/76)。肠息肉合并肠套叠的高频超声图像表现:腹部包块,短轴呈"靶环征",中央肠腔内见实质性不均匀低回声团;长轴呈"套筒征",中央肠腔内见椭圆形或类圆形不均匀低回声团,并通过蒂样低回声连接于肠壁一端;血流丰富,可见血流从肠壁通过细长条索状蒂流入低回声团内。结果表明,高频超声可清晰显示肠息肉数目、大小、血供等情况,对肠息肉的诊出率较高,且能准确诊断肠套叠的部位、范围等,对肠息肉合并肠套叠的诊断价值较高。
This study was to investigate the diagnostic value of high-frequency ultrasound(HFU)on pediatric intestinal polyposis(IP)associated with intussusception,author reviewed the patients’ data of 65 cases with that diseases treated in author’s hospital(2018-01-2019-12)who all receive HFU(color Doppler)inspection after admitted,analysed inspected rate by HFU,and image features.As results,HFU inspected out59 cases(90.77%,59/65)being of IP associated with intussusception,operation and enteroscopy showed 56 cases being of single polyp(totalled 56 pieces),9 cases of multi-polyp(totalled 20 pieces);HFU inspected 59 cases being of IP(totalled 68 pieces,including single one in 52 cases,multi-polyps in 7 cases with 15 pieces totalled,thus inspected rate was 89.47%,68/76);as for image performace,in abdominal mass,shortaxle shown "target-circle sign",and parenchymatous un-even low echo sign shown within central intestinal cavity,while,long-axle shown "thimble sign",inside central intestinal cavity appeared ellipse-like or roundkind uneven low echo sign which connected to a end of intestinal wall through the bose-like low echo,their blood flow reching,even blood flow from intestinal wall across fine long bose into low echo mass.Results show that HFU can clearly show the number,size and blood supply of polyps,its inspected rate of polyps is high,and,polyps’ accuracy site and range,etc could be inspected out,thus for IP associated intussusception has higher diagnostic value.
