摘要
目的探讨小儿肠套叠诊断中腹部超声联合浅表超声的应用价值。方法选取2016年5月~2019年5月我院收治并确诊的47例小儿肠套叠患儿作为研究对象,所有患儿分别进行腹部超声检查和浅表超声检查,比较单独腹部超声诊断和联合浅表超声的诊断的结果,以及腹部联合浅表超声在诊断不同疾病类型方面的符合率,分析超声影像表现。结果腹部联合浅表超声诊断准确率为95.74%,高于单独腹部超声的诊断的48.94%,差异有统计学意义(P<0.05)。不同疾病类型联合诊断符合率方面,小肠肠套叠均为100.00%、回盲肠肠套叠为96.55%、回结肠肠套叠为90.00%。超声影像表现方面,包块边界清晰、形态规则,回声为不均匀的低回声,纵切面可见套筒征,横切面可见同心圆征、靶环征,97.78%包块内部有棒状或点状血流信号,37.78%有肿大的淋巴结回声,2.22%有液性暗区。结论小儿肠套叠诊断中腹部超声联合浅表超声有着较高的应用价值,诊断准确率高,能够准确鉴别不同疾病类型,让患儿得到及时、有效的治疗,改善预后。
Objective To investigate the value of abdominal ultrasound combined with superficial ultrasound in the diagnosis of intussusception in children.Methods A total of 47 children with intussusception in our hospital from May 2016 to May 2019 were enrolled.All children underwent abdominal ultrasound examination and superficial ultrasound examination.The results of combined superficial ultrasound diagnosis,as well as the coincidence rate of abdominal combined superficial ultrasound in the diagnosis of different disease types,were analyzed for ultrasound imaging findings.Results The accuracy of abdominal combined with superficial ultrasound diagnosis was 95.74%,which was higher than 48.94%of the diagnosis of abdominal ultrasound alone,the difference was statistically significant(P<0.05).In terms of the combined diagnosis rate of different disease types,the small intestine intussusception was 100.00%,the ileocecal intussusception was 96.55%,and the ileal intussusception was 90.00%.In terms of ultrasonic image performance,the boundary of the mass is clear and the shape is regular.The echo is uneven and low echo.The longitudinal section shows the sleeve sign.The cross-section shows the concentric circle sign and the target ring sign.97.78%of the masses have rod-shaped or punctate blood inside.Flow signal,37.78%had swollen lymph node echo,and 2.22%had liquid dark area.Conclusion In the diagnosis of pediatric intussusception,abdominal ultrasound combined with superficial ultrasound has high application value,high diagnostic accuracy,accurate identification of different disease types,and timely and effective treatment for children to improve prognosis.
