血清LIF、HB-EGF与冻融胚胎移植术后妊娠结局的相关性及对妊娠成功预测价值分析 被引量:1

Correlation between serum LIF,HB-EGF and pregnancy outcome after frozen thawed embryo transfer and their predictive value in pregnancy success

摘要 目的:探讨血清白血病抑制因子(LIF)、肝素结合表皮生长因子样生长因子(HB-EGF)与冻融胚胎移植术后妊娠结局的相关性及对妊娠成功预测价值分析.方法:选取2019年3月至2019年12月拟进行冻融胚胎移植术患者的92个周期,随访根据妊娠结局分为妊娠组(32周期)和非妊娠组(60周期),比较两组年龄、不孕年限、子宫内膜形态、移植日子宫内膜厚度等可能影响妊娠结局的因素;比较2组移植日血清LIF、HB-EGF水平,并采用Logistic回归分析法明确可能导致妊娠失败的危险因素;通过绘制受试者工作特征曲线(ROC),分析血清LIF、HB-EGF水平对妊娠成功的预测价值.结果:妊娠组与未妊娠组子宫内膜形态、移植日子宫内膜厚度比较,差异有统计学意义(P<0.05);妊娠组移植日血清LIF质量浓度[(24.45±2.91)pg/mL]高于未妊娠组[(19.32±2.04)pg/mL],HB-EGF质量浓度[(3320.59±300.74)pg/mL]高于未妊娠组[(2430.55±261.37)pg/mL],差异均有统计学意义(P<0.05);经Logistic回归分析,子宫内膜形态、移植日子宫内膜厚度、血清LIF、HB-EGF水平均是导致冻融胚胎移植术后妊娠失败的危险因素(OR=3.074、2.563、4.437、3.881,P<0.05);经ROC曲线分析,血清LIF预测冻融胚胎移植术后妊娠成功的灵敏度、特异度、准确度和AUC分别为84.38%、76.67%、79.35%、0.760;血清HB-EGF水平预测的灵敏度、特异度、准确度和AUC分别为78.13%、73.33%、75.00%、0.722.结论:血清LIF、HB-EGF水平与冻融胚胎移植术后妊娠结局关系密切,且两者对冻融胚胎移植术后妊娠成功有较高预测价值. Objective:To explore the correlation between serum leukemia inhibitor factor(LIF),heparin-binding epidermal-growth-actor-like growth factor(HB-EGF)and pregnancy outcome after frozen thawed embryo transfer and to analyze their predictive value in pregnancy success.Methods:92 cycles of patients planning to accept frozen thawed embryo transfer from March 2019 to December 2019 were selected.The follow-up was divided into pregnancy group(32 cycles)and non pregnancy group(60 cycles)according to the pregnancy outcome.The factors possible to affect pregnancy outcome were compared in the patients of two groups,including age,infertility duration,endometrial morphology and endometrial thickness on the day of transfer.The levels of LIF and HB-EGF were also compared between the two groups.The risk factors of pregnancy failure were determined by logistic regression analysis.By drawing the receiver operating characteristic curve(ROC)of subjects,the predictive value of serum LIF and HB-EGF levels on pregnancy success was analyzed.Results:There were significant differences in endometrial morphology and endometrial thickness between pregnant group and non pregnant group(P<0.05).The serum LIF level[(24.45±2.91)pg/mL vs.(19.32±2.04)pg/mL]and HB-EGF level[(3320.59±300.74)pg/mL vs.(2430.55±261.37)pg/mL]of pregnancy group was higher than that of non pregnancy group(P<0.05)on the day of transplanation.The logistic regression analysis showed that endometrial morphology,endometrial thickness on the day of transplantation,and serum LIF and HB-EGF levels were all risk factors of pregnancy failure after frozen thawed embryo transfer(OR=3.074,2.563,4.437,3.881,P<0.05).The ROC analysis indicated that the sensitivity,specificity,accuracy and AUC of serum LIF in predicting pregnancy success after frozen thawed embryo transfer were 84.38%,76.67%,79.35%and 0.760,respectively,and the sensitivity,specificity,accuracy and AUC of serum HB-EGF were 78.13%,73.33%,75.00%and 0.722,respectively.Conclusion:Serum LIF and HB-EGF level are closely related
