妊娠期甲亢孕妇血清甲状腺功能TRAb阳性率与TSH、FT_(3)、FT_(4)、T_(3)、T_(4)水平变化的相关性分析 被引量:4

摘要 目的:研究妊娠期甲亢孕妇血清甲状腺功能促甲状腺素受体抗体(TRAb)阳性率与促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT_(3))、游离甲状腺素(FT_(4))、三碘甲状腺原氨酸(T_(3))、甲状腺素(T_(4))水平变化的相关性。方法:回顾性分析医院2018年1月-2020年2月收治的100例妊娠期甲亢孕妇纳入研究,记为观察组。同期正常妊娠孕妇100例作为对照组。检测并对比两组孕妇妊娠早、中、晚期的血清甲状腺功能TRAb阳性率与TSH、FT_(3)、FT_(4)、T_(3)、T_(4)水平,并以Pearson相关性分析明确妊娠期甲亢孕妇血清甲状腺功能TRAb阳性率与TSH、FT_(3)、FT_(4)、T_(3)、T_(4)水平的关系。结果:观察组孕早期TRAb阳性率高于对照组,且T_(3)、T_(4)水平均高于对照组(P<0.05)。观察组孕中期TRAb阳性率高于对照组,且FT_(3)、FT_(4)水平低于对照组,而T_(3)、T_(4)水平高于对照组(均P<0.05)。观察组及对照组孕晚期TRAb阳性率及TSH水平对比差异无统计学意义(P>0.05);而观察组FT_(3)、FT_(4)水平低于对照组,T_(3)、T_(4)水平高于对照组(均P<0.05)。经Pearson相关性分析可得,妊娠期甲亢孕妇血清TRAb阳性率与FT_(3)、FT_(4)均呈负相关,而与T_(3)、T_(4)水平均呈正相关(P<0.05)。结论:不同妊娠期甲亢孕妇血清甲状腺功能TRAb阳性率与FT_(3)、FT_(4)、T_(3)、T_(4)水平存在明显变化,其中TRAb阳性率与FT_(3)、FT_(4)、T_(3)、T_(4)水平密切相关,值得临床重点关注。 Objective:To study the correlation between serum thyroid function TRAb positive rate and thyroid-stimulating hormone(TSH),three free iodine thyroid former glycine(FT_(3))and free thyroxine(FT_(4)),three iodine thyroid glycine(T_(3)),thyroxine(T_(4))level in hyperthyroidism during pregnancy.Method:Retrospective analysis a total of 100 pregnant women with hyperthyroidism in pregnancy admitted to the hospital from January 2018 to February 2020 were included in the study,which were recorded as the observation group.Another 100 normal pregnant women in the same period were selected as the control group.The positive rate of serum thyroid function TRAb in early,middle and late pregnancy were compared with TSH,FT_(3),FT_(4),T_(3) and T_(4) levels in the two groups,and Pearson correlation analysis was used to determine the relationship between serum thyroid function TRAb positive rate and TSH,FT_(3),FT_(4),T_(3) and T_(4) levels in pregnant women with hyperthyroidism during pregnancy.Result:TRAb positive rate in early pregnancy in the observation group was higher than that in the control group,and T_(3) and T_(4) levels were higher than those in the control group(P<0.05).The TRAb positive rate in middle pregnancy in the observation group was higher than control group,and the FT_(3) and FT_(4) levels in the observation group were lower than control group,while the T_(3) and T_(4) levels were higher than those in the control group(P<0.05).The TRAb positive rate and TSH level in later stage of pregnancy in the observation group and the control group were not significant(P>0.05).The levels of FT_(3) and FT_(4) in the observation group were lower than control group,and the levels of T_(3) and T_(4) were higher than control group(P<0.05).Pearson correlation analysis showed that serum TRAb positive rate of pregnant women with hyperthyroidism during pregnancy was negatively correlated with FT_(3) and FT_(4),while positively correlated with T_(3) and T_(4) levels(P<0.05).Conclusion:The positive rate of TRAb in serum thyroid functi
