摘要
目的探讨预见性护理措施在老年口腔癌患者游离皮瓣移植修复术围手术期应用效果。方法选择2018年1月—2019年12月在本科行游离皮瓣移植修复术的老年口腔癌患者96例作为研究对象,随机数表法分为观察组和对照组两组,每组各48例。观察组患者行预见性护理措施,对照组行常规护理,评估两组患者术后并发症发生情况;比较手术前后体质指数(BMI)、血清白蛋白(ALB)、血清前白蛋白(PA)的变化并比较手术前后SDS、SAS的变化。结果在手术后一周,观察组患者并发症发生率8.3%,优于对照组的22.9%(P<0.05);观察组的SAS、SDS均显著低于对照组(P<0.01);体质量指数、白蛋白、前白蛋白均较对照组有明显改善(P<0.01)。结论预见性护理有利于减少老年口腔癌患者游离皮瓣移植修复术术后并发症,改善患者围手术期的营养状况及心理状态,值得借鉴。
Objective To explore the application effect of predictive nursing measures in the perioperative period of free skin flap transplantation and repair for elderly patients with oral cancer.Methods From January 2018 to December 2019,96 elderly patients with oral cancer who received free skin flap transplantation and repair in the stomatology department were selected as the research subjects.They were randomly divided into observation group and control group according to random number table method,with 48 cases in each group.The observation group received predictive nursing measures,and the control group received routine nursing,and the incidence of postoperative complications in the two groups were evaluated.The changes of body mass index(BMI),serum albumin(ALB)and serum prealbumin(PA)were compared.The changes of SDS and SAS were compared.Results At one week after operation,the complication rate of observation group was 8.3%,which was better than 22.9%of control group(P<0.05).SAS and SDS in observation group were significantly lower than those in control group(P<0.01).The body mass index,albumin and prealbumin were significantly improved compared with the control group(P<0.01).Conclusions Predictive nursing is beneficial to reduce postoperative complications of free skin flap transplantation and repair in elderly patients with oral cancer,and improve the nutritional status and psychological status of patients during perioperative period,which is worth reference.
