摘要
目的 探讨医护一体化全程管理对口腔鳞癌患者口腔卫生行为、生活质量及心理状态的影响。方法将2017年8月-2020年8月医院内接受口腔癌切除术治疗的80例患者为研究对象,按照组间基本特征具有可比性的原则分为对照组和观察组,各40例。对照组采用常规护理模式,观察组实行医护一体化全程管理模式。观察比较两组口腔卫生行为情况,焦虑(SAS)或抑郁(SDS)自评量表得分,生活质量评分。结果 医护一体化全程管理模式干预3个月,观察组口腔检查、洁牙、刷牙等口腔卫生行为优于对照组,差异有统计学意义(P<0.05);两组SDS和SAS评分均降低,观察组低于对照组,差异有统计学意义(P<0.05);3个月后观察组EORTC QLQ-H&N35各维度除牙齿及性生活外评分均低于对照组,差异有统计学意义(P<0.05)。结论 对口腔癌患者实施医护一体化全程管理干预,有效改善了其生活质量,提升其心理积极性。
Objective To investigate the effects of integrated medical management on oral health behavior,quality of life and psychological status of patients with oral squamous cell carcinoma. Methods From August 2017to August 2020, 80 patients receiving oral cancer resection in the hospital were selected as the research object and they were divided into a control group and an observation group, with 40 cases in each group according to the principle of comparability of basic characteristics between groups. The control group adopted routine nursing model, and the observation group adopted whole-process management model of integrated medical care. Oral health behavior, Self-Rating Anxiety Scale(SAS) or Self-Rating Depression Scale(SDS) score and quality of life score were observed and compared between two groups. Results After 3 months of intervention, the observation group performed better than the control group in oral health behaviors such as oral examination, dental cleaning and brushing, the differences were statistically significanct(P<0.05). SDS and SAS scores of the two groups were decreased, and the observation group was lower than the control group, the difference was statistically significant(P<0.05). After 3 months, the EORTC QLQ-H&N35 score of the observation group was lower than the control group in all dimensions except teeth and sexual life, the dif ference was statistically significant(P<0.05).Conclusion The whole process management intervention of oral cancer patients could effectively improve their life quality and psychological initiative.
