摘要
目的:了解北京大学第三医院(以下简称“我院”)住院患者厄他培南使用情况,评价用药合理性并分析原因。方法:采用回顾性研究设计、分层随机抽样法,获取2018年4月至2019年3月我院使用厄他培南的住院患者病历300份,基于《抗菌药物临床应用指导原则(2015年版)》、相关指南及专家共识,对患者使用厄他培南的适应证、微生物送检情况、用法与用量以及联合用药等方面进行统计分析,评价用药合理性及其相关影响因素。结果:使用厄他培南的300例住院患者中,围手术期预防用药42例(占14.00%);治疗用药258例(占86.00%),其中208例接受了微生物检查,治疗用药前微生物送检111例(占微生物送检总病例数的53.37%)。合理用药238例,用药合理率为79.33%;不合理用药主要表现为抗菌药物用药起点过高、用药疗程过长以及联合用药不适宜等。影响厄他培南用药合理性的因素包括是否有临床药师干预(P<0.05)和科室类别(P<0.001)。结论:我院厄他培南的使用仍存在不合理现象,需进一步加强干预管理,促进厄他培南在临床的合理应用。
OBJECTIVE:To investigate the application of ertapenem in inpatients of Peking University Third Hospital(hereinafter referred to as"our hospital"),so as to evaluate the rationality of medication and analyze the reasons.METHODS:The retrospective study method were used to obtain 300 medical records of inpatients receiving ertapenem in our hospital from Apr.2018 to Mar.2019.Based on the Guidelines for Clinical Application of Antibiotics(2015 Edition),relevant guidelines and expert consensus,statistical analysis was performed on indications,microbiological examination submission,usage and dosage,and drug combination.Rationality of medication and its related influencing factors were evaluated.RESULTS:Among the 300 inpatients receiving ertampenem,42 cases(14.00%)received perioperative prophylactic drugs.There were 258 cases(86.00%)of therapeutic drugs,among which 208 cases received microbiological examination,and 111 cases(53.37% of the total number of microbiological examination)were submitted for microbiological examination before therapeutic drugs.There were 238 cases of rational drug application,with the rational rate of 79.33%.The main manifestations of irrational drug application were high starting point of antibiotics,long duration of treatment and inappropriate drug combination.Factors influencing the application rationality of ertapenem included the clinical pharmacist intervention(P<0.05)and department types(P<0.001).CONCLUSIONS:The application of ertapenem in our hospital is still irrational,intervention management needs to be further strengthened to promote the rational application of ertapenem in clinic.
