摘要
目的:探究人乳头瘤病毒(HPV)分型和血清高迁移率蛋白A1(hmga1)、人附睾蛋白4(HE4)表达与宫颈癌患者预后关系。方法:选择2012年8月—2018年9月河南科技大学第一附属医院收治的宫颈癌患者105例(宫颈癌组),同期体检健康女性110例(对照组)。分析血清hmga1、HE4水平差异及与宫颈癌患者预后的可能关系。结果:宫颈癌组血清hmga1(168.87±18.09ng/ml)和HE4(137.18±66.43 pmol/L)水平均高于对照组(109.74±13.84 ng/ml、47.85±10.22 pmol/L)(P<0.05),不同病理类型宫颈癌患者水平无差异(P>0.05),不同FIGO分期、分化程度以及淋巴转移患者水平有差异(P<0.05)。105例宫颈癌患者检测出93.3%HPV感染,其中感染HPV1672.8%,HPV1810.5%。单因素分析显示,不同年龄、病理分期、浸润深度、淋巴转移、宫颈管直径、宫体是否累及、临床分期的患者其预后有差异(P<0.05);Cox回归分析显示,肿瘤分化、淋巴转移以及FIGO分期是影响患者预后的独立危险因素。结论:HPV分型、血清hmga1和HE4与宫颈癌有关,且宫颈肿瘤分化、淋巴转移以及FIGO分期是影响患者预后的独立危险因素。
Objective:To explore the HPV typing of the women with cervical cancer,and to study the relationship of the expressions levels of serum high mobility protein A1(HMGA 1)and human epididymal protein 4(HE 4)and the prognosis of the women.Methods:105 women with cervical cancer were selected in study group from August 2012 to September 2018.And 110 healthy women who underwent physical examination were selected in control group during the same period.The differentiation and correlation of serum HMGA 1 and HE 4 level with the prognosis of the women with cervical cancer were analyzed.Results:The serum HMGA 1 level(168.87±18.09 ng/mL)and HE4 level(137.18±66.43 pmol/L)of the women in the study group were significant higher than those(109.74±13.84 ng/mL and 47.85±10.22 pmol/L)of the women in the control group(P<0.05).There were no significant difference in the levels of serum HMGA 1 and HE 4 among the women with different pathological types of cervical cancer(P>0.05),but which had significant differences among the women with different FIGO stage,among the women with different degree of differentiation,and the women with different lymphatic metastasis(P<0.05).HPV infection was detected in 105(93.3%)women with cervical cancer,and among which,HPV 16 infection rate was 72.8%and HPV 18 infection rate was 10.5%.Univariate analysis showed that there were significant differences in the prognosis of the women with different ages,pathological stages,infiltration depth,lymphatic metastasis,cervical canal diameter,uterine involvement,or clinical stages(P<0.05).Cox regression analysis showed that tumor differentiation,lymphatic metastasis,and FIGO stage of the women were the independent risk factors affecting their prognosis.Conclusion:HPV typing and the serum HMGA 1 and HE4 levels of the women are associated with their cervical cancer occurrence,and tumor differentiation,lymphatic metastasis,and FIGO stage of the women with cervical cancer are the independent risk factors affecting their prognosis.
