摘要
目的分析和阐述PDCA循环法在平均住院日管理中的实际应用成果。方法采集数据库中一定时期内所有出院病人病案首页的信息字段作为基础数据。通过计划-执行-检查-行动的顺序进行持续质量改进。设定平均住院日的目标值,制定和落实各项改进措施,运用R语言3.6. 1版本进行数据统计分析,并对改进的结果进行总结。结果以平均住院日为应变量,可能的影响因素为自变量,进行多元线性回归分析,回归模型具有统计学意义(F=10.437,P <0.05)。结果显示术前待床日和CMI对平均住院日的影响具有统计学意义(P <0.05)。住院天数超过30天的患者共1261人次,占全院出院人次的1.53%。2019全年平均住院日达成预期目标,各月数据均低于历史同期水平。结论应用PDCA循环法管理平均住院日的成果显著,合理控制平均住院日可以实现医疗质量和医疗效率的同步提升。
Objective To analyze the practical application of PDCA method in management of average hospital stay.Methods The medical records of all discharged patients in a certain period were collected as the basic data.Through Plan-DoCheck-Action measures to implement the continuous improvement of service quality.To set the target of average hospital stay and to formulate effective improvement measures,R language version 3.6.1 was used for data statistical analysis,and the improved results were summarized.Results Multiple linear regression analysis was conducted with the average length of hospitalization as the dependent variable and the possible influencing factors as the independent variable.The regression model was statistically significant(F=10.437,P<0.05).The results showed that the preoperative length of stay and CMI had significant impact on average length of stay(P<0.05).A total of 1261 patients were hospitalized for more than 30 days,accounting for 1.53%of the total of the discharged.The actual average length of hospitalization in 2019 reached the expected target,and the monthly data were all lower than the historical level of the same period.Conclusion The application of PDCA method to manage the length of hospitalization has remarkable achievements.The reasonable management of the average hospital stay can realize the synchronous improvement of medical quality and medical efficiency.
