乳腺癌液实性区表观扩散系数与分子亚型及免疫组化因子的相关性研究 被引量:3

摘要 目的:探讨乳腺癌液实性区表观扩散系数(ADC)与分子亚型、免疫组化因子的相关性,分析其判断乳腺癌分子亚型的应用价值。方法:对广西医科大学附属肿瘤医院2017年1月至2019年12月经病理证实存在液实性区的60例乳腺癌患者进行回顾性分析。分别测量瘤灶液性区、实性区的ADC值(ADC_(液性区)及ADC_(实性区)),并计算两者的比值(ADC比值=ADC_(液性区)/ADC_(实性区))。比较不同分子亚型乳腺癌中ADC液性区、ADC实性区以及ADC比值之间的差异。采用Spearman相关性分析评估上述ADC值与免疫组化因子Ki-67、人表皮生长因子受体2(HER-2)、雌激素受体(ER)、孕激素受体(PR)相关性。结果:在4种分子亚型中,ADC_(实性区)及ADC比值组间差异有统计学意义(均P<0.005);ADC_(液性区)的组间比较,差异无统计学意义(P>0.05)。相关性分析显示:ADC_(液性区)与Ki-67呈弱正相关关系(r=0.343,P=0.007),与HER-2呈弱负相关关系(r=-0.262,P=0.043),与ER呈弱负相关关系(r=-0.343,P=0.007)。ADC_(实性区)与Ki-67呈弱负相关关系(r=-0.329,P=0.010),与HER-2呈弱正相关关系(r=0.391,P=0.002),与PR呈弱负相关关系(r=-0.298,P=0.021);ADC比值与Ki67呈中度正相关关系(r=0.453,P<0.001),与HER-2呈中度负相关关系(r=-0.449,P<0.001)。结论:ADC_(实性区)与ADC比值对乳腺癌不同分子亚型判断有一定参考意义。 Objective:To explore the correlation between apparent diffusion coefficient(ADC)value,immunohistochemical factors and molecular subtypes of breast cancer in order to determine the feasibility of ADC for the detection of molecular subtypes.Methods:A total of 60 subjects who were pathologically confirmed as breast cancer in our hospital from January 2017 to December 2019 were enrolled.The ADC values of fluid area(ADCfluid)and solid area(ADCsolid)were measured,and ADC ratio(ADCfluid/ADCsolid)was calculated.The differences between ADCfluid,ADCsolid and ADC ratio in different molecular subtypes of breast cancer were compared.Spearman correlation analysis was used to evaluate the correlation between the above ADC values and Ki-67,human epidermal growth factor receptor-2(HER-2),estrogen receptor(ER),and progestogerone receptor(PR).Results:Among molecular subtypes,there were statistically significant differences in ADCsolid and ADC ratio between groups(P<0.05),but no statistically significant difference between groups in ADCfluid(P>0.05).Correlation analysis showed ADCfluid was correlated with Ki-67(r=0.343,P=0.007),HER-2(r=-0.262,P=0.043),and ER(r=-0.343,P=0.007).ADCs was correlated with Ki-67(r=-0.329,P=0.010),HER-2(r=0.391,P=0.002)and PR(r=-0.298,P=0.021).ADC ratio was correlated with Ki67(r=0.453,P<0.001),HER-2(r=-0.449,P<0.001).Conclusion:ADCsolid and ADC ratio might be a potential reference in differentiating molecular subtypes of breast cancer.
