探讨呼出气一氧化氮测定联合痰细胞计数评估哮喘表型 被引量:3

Study on the Evaluation of Asthma Phenotype by the Measurement of Exhaled Nitric Oxide Combined With the Count of Sputum Cells

摘要 目的研究支气管哮喘患者呼出气一氧化氮(FeNO)测定联合痰细胞计数评估哮喘表型,为临床治疗提供参考。方法整理收集研究对象资料,共计122例,均为2018年3月-2019年4月在本医院呼吸科接受治疗的支气管哮喘患者,通过FeNO测定联合痰细胞计数分类评估所有患者的哮喘表型,针对组间患者对比炎症标志物和肺通气功能检查指标。结果甲组与乙组的呼出气一氧化氮水平及痰嗜酸粒细胞均显著高于其他两组,表明两组患者均存在嗜酸粒细胞气道炎症特征(Th2细胞优势型)。结论呼出气一氧化氮水平属于无创标志物,是临床对支气管哮喘患者的炎症与嗜酸性粒细胞进行监测的主要依据。尤其对于重症哮喘评估预后有更重要的意义,具有良好的应用前景。 Objective To study the determination of exhaled nitric oxide(FeNO)combined with sputum cell count in patients with bronchial asthma to assess the phenotype of asthma,so as to provide reference for clinical treatment.Methods The data of the study subjects were sorted and collected,a total of 122 cases,all of which were patients with bronchial asthma who were treated in the respiratory department of our hospital from March 2018 to April 2019.The asthma phenotype of all patients was assessed by FeNO measurement combined with sputum cell count classification,comparing inflammatory markers and pulmonary ventilation function test indicators for patients between groups.Results The levels of exhaled nitric oxide and sputum eosinophils in group A and group B were significantly higher than those of the other two groups,indicating that both groups of patients had eosinophil airway inflammation characteristics(Th2 cell dominant type).Conclusion The level of exhaled nitric oxide is a non-invasive marker,and it is the main basis for clinical monitoring of inflammation and eosinophils in patients with bronchial asthma.Especially for the evaluation of prognosis of severe asthma,it has more important significance,it has good application prospects.
