摘要
目的根据哮喘儿童中痰细胞学参数与临床表现的相关性,探讨痰细胞学检查对儿童哮喘的临床意义。方法选取2018年9月至2019年9月海口市人民医院收治的确诊持续性哮喘患者共100例,其中轻度持续组(50例)、中度持续组(50例)。检测痰细胞学参数、哮喘五项临床指标和肺功能;采用Spearman分析痰细胞学参数与哮喘五项临床指标变化检测之间的关联性。结果哮喘儿童治疗前中度持续组嗜酸性粒细胞比例高于轻度持续组(P<0.01),而两组巨噬细胞、中性粒细胞和淋巴细胞比例相似;治疗后,两组嗜酸性粒细胞、巨噬细胞和淋巴细胞比例均降低(P均<0.05),中性粒细胞比例均升高(P均<0.05),且两组各痰细胞百分比相似;在治疗前、后,轻度持续组FEV1和FEV1/FVC比值均高于中度持续性组(P均<0.05),痰细胞学嗜酸性粒细胞比例与急性加重天数、使用急救药物沙丁胺醇、急诊就诊次数、夜间咳嗽、缺课天数呈正相关,与FEV1和FEV1/FVC呈负相关(P均<0.05)。结论痰中嗜酸性粒细胞减少与临床哮喘参数和FEV1的改善相关。
Objective To explore the clinical significance of sputum cytology in children with asthma.Methods A total of 100 patients with mild and moderate persistent asthma were admitted to our hospital from September 2018 to September 2019 were selected,including mild persistent group(50 cases)and moderate persistent group(50 cases).The sputum cytology test parameters,sputum cytology test parameters,five clinical indicators of asthma,and pulmonary function were detected,the correlation between the sputum cytology parameters and the detection of changes in five clinical indicators of asthma was analyzed using Spearman.Results The percentage of eosinophils in the moderate sustained group was significantly higher than that in the mild sustained group(P<0.01),and the proportion of macrophages,neutrophils and lymphocytes in the two groups was similar.After 3 months of treatment,the percentages of eosinophils,macrophages,and lymphocytes were significantly reduced(P all<0.05),and the percentage of neutrophils was significantly increased(P all<0.05),and the percentages of sputum cells in the two groups were similar.Before and after treatment,the ratios of FEV1 and FEV1/FVC in the mild persistent group were significantly higher than those in the moderate persistent group(P all<0.05).The proportion of eosinophils in sputum cytology was positively correlated with the number of days of acute exacerbation,the use of emergency medicine salbutamol,the number of emergency visits,night cough,days of absence from school,and negatively correlated with FEV1 and FEV1/FVC(P all<0.05).Conclusion The reduction of eosinophils in sputum is related to the improvement of clinical asthma parameters and FEV1.
