摘要
目的探究双相障碍抑郁发作患者外周血超敏C反应蛋白(hs-CRP)与认知功能之间的相关性。方法选择2019年2月至2020年2月于丽水市第二人民医院门诊或住院的首诊或复发未用药的双相抑郁患者作为研究组(24例),经过匹配后非双相抑郁症患者为阳性对照组(24例),健康人群为阴性对照组(24例)。收集各组一般情况、认知功能、生化检验等资料,采用Sprearman回归分析血浆hs-CRP与双相障碍抑郁发作患者认知功能的相关性。结果研究组精神分裂症认知功能成套测验(MCCB)评分为(40.37±4.18)分,低于阳性对照组[(44.37±4.07)分]和阴性对照组[(49.72±3.27)分],三组差异有统计学意义(F=8.376,P<0.05);研究组hs-CRP、白细胞介素6(IL-6)水平高于阳性对照组和阴性对照组,三组差异均有统计学意义(F=10.273、7.378,均P<0.05),且阳性对照组hs-CRP、IL-6水平均高于阴性对照组(t=3.380、2.214,均P<0.05)。Spearman相关性分析显示,hs-CRP(r=-0.56,P<0.05)、IL-6(r=-0.34,P<0.05)水平与双相障碍抑郁发作患者MCCB评分呈负相关。结论双相障碍抑郁患者认知功能下降,hs-CRP等炎性因子升高,且炎性因子水平与认知功能呈负相关。
Objective To correlate high sensitivity C-reactive protein with cognitive function in patients with bipolar depression.Methods Twenty-four patients with bipolar depression without medication who received treatment in the clinics and wards of the Second People's Hospital of Lishui from February 2019 to February 2020 were included in the study group.Twenty-four matched patients with non-bipolar depression who received treatment concurrently were included in the positive control group.Additional 24 healthy subjects were included in the negative control group.General situation,cognitive function and biochemical test data were collected from each group.Spearman's rank correlation was performed to analyze the correlation between serum high sensitivity C-reactive protein and cognitive function in patients with bipolar depression.Results The MATRICS Consensus Cognitive Battery score in the study group was significantly lower than that in the positive control and negative control groups[(40.37±4.18)points vs.(44.37±4.07)points or(49.72±3.27)points,F=8.376,P<0.05].High sensitivity C-reactive protein and interleukin-6 levels in the study group were significantly higher than those in the positive control and negative control groups(F=10.273,7.378,both P<0.05).High sensitivity C-reactive protein and interleukin-6 levels in the positive control group were significantly higher than those in the negative control groups(t=3.380,2.214,both P<0.05).Spearman correlation analysis showed that high sensitivity C-reactive protein(r=-0.56,P<0.05)and interleukin-6(r=-0.34,P<0.05)levels were negatively correlated with the MATRICS Consensus Cognitive Battery score in patients with bipolar depression.Conclusion In patients with bipolar depression,cognitive function is decreased,the levels of high sensitivity C-reactive protein and other inflammatory factors are increased,and the levels of inflammatory factors are negatively correlated with cognitive function.
