2型糖尿病患者血清胰高血糖素样肽-1水平与轻度认知功能障碍的相关性研究 被引量:10

摘要 目的分析2型糖尿病患者血清胰高血糖素样肽-1(glucagon-like peptide-1,GLP-1)水平与轻度认知功能障碍(mild cognitive impairment,MCI)的相关性。方法选择2019年2月至2020年6月于应急总医院内分泌科或神经内科就诊的80例2型糖尿病患者,采用蒙特利尔认知评估量表(Montreal cognitive assessment,MoCA)测评其认知功能,其中2型糖尿病合并MCI患者36例,作为研究组,2型糖尿病认知功能正常者44例,作为糖尿病对照组,选择2019年3月至2020年7月应急总医院体检中心的健康体检者51例作为正常对照组。所有受试者行一般生化指标检测,行口服葡萄糖耐量试验,测定空腹及0.5、1、2h的血糖(blood glucose,BG)、胰岛素(insulin,INS)、C肽(C-peptide,C-P)、GLP-1水平,计算曲线下面积(the area under the curve,AUC),采用多元线性回归分析2型糖尿病患者MCI的危险因素。结果研究组总胆固醇、甘油三酯、糖化血红蛋白水平高于糖尿病对照组及正常对照组,差异有显著性(P<0.05)。与正常对照组比较,研究组和糖尿病对照组AUCBG明显升高,AUCINS、AUCC-P明显降低,差异有显著性(P<0.05);与糖尿病对照组及正常对照组比较,研究组口服葡萄糖耐量试验各阶段GLP-1水平及AUCGLP-1均明显降低(P<0.05)。多元线性回归分析显示糖尿病病程、C-P(空腹)、GLP-1(空腹)、BG(空腹)、HbA1c对2型糖尿病患者的MCI具有预测作用。结论2型糖尿病合并MCI患者血清GLP-1水平明显降低,对其进行监测可能有助于2型糖尿病患者MCI的早期诊断、早期干预。 Objective To analyze the correlation between serum glucagon-like peptide-1(GLP-1)levels and mild cognitive impairment in patients with type 2 diabetes mellitus.Method 80 patients with type 2 diabetes mellitus who were admitted to the Department of Endocrinology or Neurology of Emergency General Hospital from February 2019 to June 2020 were recruited.The Montreal Cognitive Assessment Scale(MoCA)was used to measure cognitive function,and the patients were divided into groups according to the MoCA score,36 cases of type 2 diabetes mellitus with mild cognitive impairment group(study group)and 44 cases of type 2 diabetes mellitus with normal cognitive function group(control group),and 51 healthy subjects in the physical examination center from March 2019 to July 2020 in the Emergency General Hospital served as normal controls(NC group).All subjects underwent general biochemical index testing,oral glucose tolerance test(OGTT test),blood glucose,insulin,C-peptide,and GLP-1 levels were measured,calculate the area under the curve of blood glucose(AUCBG),the area under the curve of insulin(AUCINS),the area under the curve of C peptide(AUCC-P),and the area under the curve of GLP-1(AUCGLP-1),multiple linear regression analysis was used to explore the relationship between GLP-1 level and MoCA score.Result TC,TG,HbA1c in the study group were significantly higher than those in the control group and the NC group(P<0.05),compared with the NC group,the AUCBG in the study group and the control group increased significantly(P<0.05).AUCINS and AUCC-P were significantly reduced(P<0.05).Compared with the control group and NC group,the GLP-1 levels and AUCGLP-1 in each segment of the OGTT test in the study group were significantly lower(P<0.05).Multivariate linear correlation analysis showed that the course of diabetes,C-P(fasting),GLP-1(fasting),BG(fasting),and HbA1c had predictive effects on mild cognitive impairment.Conclusion Monitoring the level of GLP-1 in patients with type 2 diabetes mellitus is helpful for early diagnosis and e
