经支气管镜冷冻治疗气管支气管结核的有效性及安全性的Meta分析 被引量:4

Meta-analysis of the Effectiveness and Safety of Bronchoscopy Cryotherapy for Tracheobronchial Tuberculosis

摘要 目的系统评价经支气管镜冷冻治疗气管支气管结核的有效性及安全性。方法计算机检索中国知网、维普、万方、中国生物文献数据库、PubMed、EMBASE、The Cochrane Library、Web of Science,收集从建库至2020年2月24日关于冷冻治疗气管支气管结核的随机对照研究。按照纳入与排除标准筛选文献、提取资料并对文献进行评价质量后,采用RevMan5.3软件进行Meta分析。结果共纳入研究13个,合计患者1298例。Meta分析显示,冷冻治疗气管支气管结核总有效率优于常规抗结核化疗[RR=1.23,95%CI=(1.17,1.29),P<0.05];冷冻治疗气管支气管结核痰菌转阴率优于常规抗结核化疗[RR=1.32,95%CI=(1.09,1.61),P=0.005];冷冻治疗不良反应发生率低于常规抗结核化疗[RR=0.38,95%CI=(0.24,0.62),P=0.0001]。结论经支气管镜冷冻治疗气管支气管结核在总有效率、痰菌转阴率与降低不良反应方面优于常规抗结核化疗,但结果还需进一步研究证实。 Objective To systematically evaluate the effectiveness and safety of bronchoscopy cryotherapy for tracheobronchial tuberculosis.Methods Computer search of CNKI,Weipu,Wanfang,China Biological Literature Database,PubMed,EMBASE,The Cochrane Library,Web of Science,collected randomized controlled studies on cryotherapy for tracheobronchial tuberculosis from the establishment of the database to February 24,2020.After screening the literature according to the inclusion and exclusion criteria,extracting the data and evaluating the quality of the literature,the RevMan5.3 software was used for Meta analysis.Results A total of 13 studies were included,with a total of 1298 patients.Meta analysis showed that the total effective rate of cryotherapy for tracheobronchial tuberculosis was better than conventional anti-tuberculosis chemotherapy[RR=1.23,95%CI=(1.17,1.29),P<0.05];the sputum conversion rate of cryotherapy for tracheobronchial tuberculosis was better than conventional anti-tuberculosis chemotherapy[RR=1.32,95%CI=(1.09,1.61),P=0.005];the incidence of adverse reactions of cryotherapy was lower than that in conventional anti-tuberculosis chemotherapy[RR=0.38,95%CI=(0.24,0.62),P=0.0001].Conclusion Bronchoscopy cryotherapy for tracheobronchial tuberculosis is superior to conventional anti-tuberculosis chemotherapy in terms of total effective rate,sputum conversion rate and reduction of adverse reactions,but the results need to be confirmed by further studies.
