摘要 目的探讨吲哚菁绿(ICG)荧光实时成像联合术中超声技术在特殊部位腹腔镜肝癌切除术中的临床应用价值。方法回顾性分析2018年1月至2020年5月绍兴市人民医院应用ICG荧光实时成像联合术中超声技术的特殊部位腹腔镜肝癌切除术20例患者的临床资料。结果20例患者仅1例中转开腹;19例完成腹腔镜肝癌切除术,其中ICG荧光显像成功17例,失败2例,1例为为胆管细胞癌,1例为混合型肝癌。19例均在腹腔镜下完整切除病灶,切缘均阴性,手术时间(165±38)min,术中出血量(280±26)mL,术后发生胆漏1例,无再出血发生。结论吲哚菁绿荧光实时成像联合术中超声技术在特殊部位腹腔镜肝癌切除术中安全、有效,值得临床进一步推广。 Objective To investigate the clinical application value of indocyanine green(ICG)fluorescent real-time imaging combined with intraoperative ultrasound on laparoscopic hepatectomy for special surgical sites.Methods Clinical data of 20 patients,who underwent laparoscopic hepatectomy from Jan.2018 to May 2020 with the help of ICG fluorescent real-time imaging combined with intraoperative ultrasound at Shaoxing People’s Hospital,were collected and analyzed.Results Only 1 of the 20 patients was converted to laparotomy;19 patients completed laparoscopic hepatectomy,of whom 17 were successful in ICG fluorescence imaging,2 failed,1 patient was confirmed to be cholangiocarcinoma,and 1 was mixed liver cancer.The lesions were completely resected in 19 cases under laparoscopic surgery,and the margins were negative.The operation time was(165±38)min,the intraoperative blood lose was(280±26)mL.Postoperative bile fistula occurred in 1 case,no rebleeding occurred.Conclusion Indocyanine green fluorescent real-time imaging combined with intraoperative ultrasound is safe and effective for laparoscopic hepatectomy,which is worthy of further clinical application.
