充气式保温毯的保温模式对腹腔镜下肝癌切除术患者保温效果的影响 被引量:6

摘要 目的比较充气式保温毯采用身上型与身下型两种保温模式对腹腔镜下肝癌切除术患者术中的保温效果。方法纳入我院2020年1月~2020年12月腹腔镜下肝癌切除术患者80例,按照随机数字表对照法分为充气式保温毯身上型组(对照组)和充气式保温毯身下型组(试验组),40例/组。两组均应用加速康复外科模式的综合保温措施:包括均在同等室温下进行手术;均使用同等型号体液加温仪,冲洗液同一拿取恒温箱的灭菌注射用水,可测温尿管检测中心体温等。对比分析两组患者术中低体温发生率、压力性损伤发生率、术后苏醒时间、术后排气时间,探讨充气式保温毯身上型和充气式保温毯身下型的应用效果。结果与对照组相比,试验组患者术中低体温发生率、压力性损伤发生率、手术部位感染发生率更低(P<0.05);试验组术后苏醒时间明显缩短(P<0.05)。两组术后排气时间的差异无统计学意义(P>0.05)。结论对腹腔镜肝癌切除术患者采用多模式保温过程中,充气式保温毯身下型的保温效果优于充气式保温毯身上型的保温效果,能有效预防低体温,压力性损伤、手术部位感染促进术后快速康复,为临床保温毯的使用提供参考数据。 Objective To compare the thermal insulation effects of the on-body inflatable thermal insulation blanket and the under-body mode during the operation.Methods We included 80 patients undergoing laparoscopic liver cancer resection in our hospital from January 2020 to December 2020.Patients were randomly divided into on-body and under-body air-type insulation blankets(40 cases/group).All patients performed with the comprehensive insulation measures,including body fluid warming apparatus,warming washing fluid,etc.We compared the low body temperature,postoperative wake-up time,postoperative exhaust time and the incidence of pressure ulcers were compared between the two groups of patients.The application effects of on-body air-type insulation blanket and under-body air-type insulation blanket were compared and analyzed.Results Compared with the use of on-body air-type insulation blankets,the incidence of hypothermia and pressure ulcers during operation was significantly reduced in patients with under-body air-type insulation blankets(P<0.05).Patients'recovery time was significantly reduced(P<0.05).There was no significant difference in the postoperative exhaust time between the two groups(P>0.05).Conclusion In the process of using multi-mode thermal insulation for patients undergoing laparoscopic liver cancer resection,the thermal insulation effect of the under-body air-type insulation blanket is better than that of the on-body air insulation blanket,which is helpful to guide the improvement of intraoperative multi-mode insulation measures and is worthwhile promotion.
