摘要
目的:观察补阳还五汤在老年股骨转子间骨折关节置换术围手术期中的应用效果。方法:选取42例老年股骨转子间骨折患者,按随机数字表法分对照组和治疗组各21例。2组均行关节置换术治疗,对照组围手术期实施快速康复外科管理,治疗组在对照组基础上予以补阳还五汤治疗。比较2组术后视觉模拟评分法(VAS)评分、髋关节功能评分(Harris评分)、并发症发生率、住院时间及住院费用。结果:术后6 h、3 d、7 d,2组VAS评分均较术前明显降低(P<0.05)。术后3 d、7 d,治疗组VAS评分均明显低于同期对照组(P<0.05)。术后7 d、30 d,2组Harris评分均较术后当天明显升高(P<0.05),治疗组Harris评分均明显高于同期对照组(P<0.05)。治疗组并发症发生率为9.52%,与对照组的19.05%比较,差异无统计学意义(P>0.05)。治疗组住院时间明显短于对照组(P<0.05)。2组住院费用比较,差异无统计学意义(P>0.05)。结论:补阳还五汤能加快老年股骨转子间骨折关节置换术患者术后髋关节功能恢复,减轻疼痛,缩短住院时间,安全性好。
Objective:To observe the applicational effect of Buyang Huanwu tang on the elderly patients who had undergone joint replacement for intertrochanteric fracture at perioperative period.Methods:A total of 42 elderly patients with intertrochanteric fracture were divided into the control group and the treatment group according to the random number table method,21 cases in each group.Both groups received joint replacement;the control group additionally implemented the surgical management of rapid rehabilitation,and the treatment group additionally received Buyang Huanwu tang based on the treatment of the control group.After surgery,the score of Visual Analogue Scale(VAS),Harris Hip Function Score(Harris score),incidence of complications,length of hospital stay and hospitalization costs were compared between the two groups.Results:Six hours,three days and seven days after surgery,the VAS scores in the two groups were significantly decreased when compared with those before surgery(P<0.05).Three days and seven days after surgery,the VAS scores in the treatment group were significantly lower than those in the control group(P<0.05).Seven days and thirty days after surgery,the Harris scores in the two groups were significantly increased when compared with those on the day after surgery(P<0.05),and the Harris score in the treatment group was significantly higher than that in the control group(P<0.05).The complication rate was 9.52%in the treatment group and 19.05%in the control group,there being no significant difference(P>0.05).The length of hospital stay in the treatment group was significantly shorter than that in the control group(P<0.05).There was no significant difference in the hospitalization costs between the two groups(P>0.05).Conclusion:Buyang Huanwu tang can accelerate the recovery of hip function,reduce pain,shorten hospital stay,and improve safety in the elderly patients who had undergone joint replacement for intertrochanteric fracture at perioperative period.
