摘要
目的探讨非肌层浸润性膀胱癌(NMIBC)合并良性前列腺增生(BPH)患者接受同期经尿道手术的治疗效果。方法回顾性选取2016年1月~2017年12月湖南中医药高等专科学校附属第一医院诊治的96例NMIBC合并BPH患者的病例资料,其中46例同期经尿道电切术治疗患者为研究组,50例仅接受经尿道膀胱肿瘤电切术(TURBT)患者为对照组。比较两组手术资料、术后复发率和血清癌胚抗原(CEA)和糖类抗原125(CA125)水平变化。结果研究组手术耗时、手术失血量、住院时间和尿管留置时间明显高于对照组,差异有统计学意义(P<0.05);研究组术后IPSS评分、PVR均低于对照组,而Qmax和QOL评分则显著高于对照组,差异有统计学意义(P<0.05)。术后3个月,研究组患者血清CEA、CA125水平明显低于对照组,差异有统计学意义(P<0.05)。术后2年研究组的复发率显著低于对照组,差异有统计学意义(P<0.05)。结论在NMIBC合并BPH患者治疗过程中,采用同期经尿道电切术虽然创伤较大,但能够明显改善前列腺症状和降低血清CEA、CA125水平。
Objective To explore the efficacy of simultaneous transurethral resection in treatment of non-muscle invasive bladder cancer(NMIBC)with benign prostatic hyperplasia(BPH). Methods Ninety-six patients with NMIBC and BPH were selected as the study objects.46 patients were treated with simultaneous transurethral resection as the study group and 50 patients were treated with transurethral resection of bladder tumor(TURBT)only as the control group.Postoperative recurrence rate and serum CEA and CA125 levels were compared between two groups. Results The operation time,blood loss,hospital stay and catheter indwelling time in the study group were significantly higher than those in the control group(P < 0.05).IPSS scores,PVR and QOL scores in the study group were all lower than those in the control group,while Qmax scores were significantly higher than those in the control group(P <0.05).The recurrence rate of 2 years after operation in the study group was significantly lower than that in the control group(P <0.05).Three months after the operation,the serum levels of CEA and CA125 in the study group were significantly lower than those in the control group(P < 0.05). Conclusion In the treatment of NMIBC and BPH,transurethral resection has more traumatic,but it can significantly improve prostate symptoms and reduce serum CEA and CA125 levels.
