摘要
目的:探讨超声乳化联合房角分离术对青光眼合并白内障患者眼压、视力、中央前房深度改善的影响。方法:选择2018年2月至2019年7月在我院接受治疗的青光眼合并白内障患者64例进行研究。采用随机数字表法将患者分为2组,每组各32例。对照组患者行超声乳化联合小梁切除术,观察组患者行超声乳化联合房角分离术。于术后1 M复查眼压、视力,并采用超声生物显微镜测量中央前房深度差值、房角开放距离500差值、小梁睫状体差值以评价疗效,同时观察术后并发症发生情况。结果:观察组总有效率明显高于对照组(P<0.05)。治疗后两组患者的眼压均明显下降,视力均明显提高,(P<0.05),但观察组效果更加明显(P<0.05)。观察组患者中央前房深度差值、房角开放距离500差值、小梁睫状体差值均明显高于对照组(P<0.05)。对照组共出现3例(9.38%)并发症,观察组共出现1例(3.13%)并发症,两组差异无统计学意义(P>0.05)。结论:超声乳化联合房角分离术较超声联合小梁切除术可明显提高青光眼合并白内障患者疗效,降低患者眼压、改善视力,提高中央前房深度差值、房角开放距离500差值、小梁睫状体差值。
Objective:To investigate the effect of phacoemulsification combined with angle separation on intraocular pressure,vision and central anterior chamber depth improvement in patients with glaucoma and cataract.Methods:From February 2018 to July 2019,64 patients with glaucoma and cataract treated in our hospital were studied.The patients were divided into control group and observation group with 32 cases in each group.The control group received phacoemulsification combined with trabeculectomy,and the observation group received phacoemulsification combined with angle separation.One month after operation,intraocular pressure(measured by intraocular pressure meter),visual acuity(measured by standard logarithmic visual acuity chart),ultrasound biomicroscopy(difference of central anterior chamber depth,angle opening distance 500,trabecular ciliary body)and postoperative complications were compared between the two groups.Results:The total effective rate of the observation group was significantly higher than that of the control group(P<0.05).After treatment,the intraocular pressure of the two groups of patients decreased significantly and their visual acuity increased significantly(P<0.05),but the observation group decreased or increased more(P<0.05).The difference of central anterior chamber depth,angle opening distance and trabecular ciliary body in the observation group were significantly higher than those in the control group(P<0.05).There were 3 cases(9.38%)in the control group and 1 case(3.13%)in the observation group.There was no significant difference between the two groups(P>0.05).Conclusion:Phacoemulsification combined with angle separation can significantly improve the efficacy of glaucoma combined with cataract,reduce intraocular pressure and improve vision and the difference of central anterior chamber depth,angle opening distance 500 and trabecular ciliary body.
