摘要
以资源消耗为基础的相对价值评价体系(RBRVS)在我国公立医院有三种主要应用模式(点值模式、绩效费率模式、本土化绩效指标模式)。随着RBRVS应用的拓展,在本土化应用过程中出现了一些问题,最终体现在RBRVS的具体计量模型上,影响绩效奖金的核算。本文基于RBRVS本土化应用的一般计量模型,结合其它绩效考评工具和辅助措施,兼顾质量、成本、效益指标,形成以工作量为基础、价值为导向、切合公立医院实际的综合绩效奖金计量模型,有利于强化医院的公益性以及提高精细化管理水平。
Relative value evaluation system(RBRVS)based on resource consumption,has three main application modes in Chinese public hospitals:point model,performance rate model and localized performance index model.With the expansion of the application of RBRVS,there are some problems in the process of localization,which are finally reflected in the specific measurement model of RBRVS and affects the accounting of performance bonus.Based on the general measurement model of localization application of RBRVS,combined with other performance evaluation tools and auxiliary measures and taking into account the indicators of quality,cost and benefit,this paper forms a comprehensive performance bonus measurement model based on workload,value-oriented and fit with the reality of public hospitals,which is helpful to strengthen the public welfare of the hospital and improve the level of fine management.
