摘要
目的:观察超声引导下真空辅助旋切治疗乳腺良性肿瘤的临床疗效。方法:回顾性分析2018年1月至2019年1月广东省东莞市东部中心医院收治的90例乳腺良性肿瘤患者的临床资料,根据其手术方式不同分为研究组(超声引导下真空辅助旋切术,46例)及对照组(传统切除手术,44例)。比较两组手术情况、应激反应[胰岛素、皮质醇(Cor)]、美观程度、并发症发生情况、残留及复发情况。结果:研究组术中出血量、手术时间、瘢痕长度、住院时间、切口愈合时间均低于对照组(P<0.05)。术后1周,研究组胰岛素、Cor水平低于对照组(P<0.05)。术后6个月,研究组乳房形状、色素沉着、切口瘢痕和乳头乳晕感觉评分均高于对照组(P<0.05)。两组并发症发生率、残留及复发情况比较,差异无统计学意义(P>0.05)。结论:超声引导下真空辅助旋切治疗乳腺良性肿瘤效果良好,能够有效改善术后应激反应,提高美观程度,有利于患者康复。
Objective:To investigate the clinical efficacy of ultrasound-guided vacuum assisted rotational lumpectomy for benign breast tumors.Methods:A retrospective analysis was performed on the clinical data of 90 patients with benign breast tumors admitted to Eastern Dongguan Central Hospital,Guangdong,between January 2018 and January 2019.According to surgical options,the patients were assigned to the study group(undergoing ultrasound-guided vacuum assisted rotational lumpectomy,n=46)and the control group(undergoing conventional resection,n=44).The surgery,stress response[as measured by levels of insulin and cortisol(Cor)],aesthetics,complications,residual lesion and recurrence were compared between the two groups.Results:The intraoperative blood loss,operation time,scar length,hospital stay,and time to incision healing in the study group were less or shorter than those in the control group(P<0.05).At one week after surgery,the levels of insulin and Cor in the study group were lower than those in the control group(P<0.05).At six months after surgery,the scores of breast shape,pigmentation,incision scar and nipple/areola sensation in the study group were higher than those in the control group(P<0.05).There was no significant difference in complications,residual lesion and recurrence between the two groups(P>0.05).Conclusion:Ultrasound-guided vacuum-assisted rotational lumpectomy offers good treatment efficacy in benign breast tumors with effective improvements in postoperative stress and aesthetics,and therefore is conducive for patient recovery.
