摘要
目的观察人工智能三维重建技术在肺小结节单孔胸腔镜肺段精准切除术前定位中的临床价值。方法选取2020年1~6月我院行单孔胸腔镜肺段精准切除手术治疗的50例肺小结节患者的临床资料,均采用人工智能三维重建技术进行定位,分析手术结果、术前定位的成功率、平均操作时间和并发症发生率。结果 50枚肺小结节的术前定位中有48例定位成功,成功率率为96.00%,均在当天内接受单孔胸腔镜肺段精准切除手术,无中转开胸手术病例,所有术后均获得明确的病理诊断结果。定位操作时间16~27 min,平均定位操作时间18.23 min;术中出血量53~66 ml,平均出血量55.31 ml;住院时间8~25 d,平均住院时间15.21 d。有4例发生无症状气胸,发生率为8.00%,未发生咯血、血胸等并发症。结论利用人工智能三维重建技术术前定位肺小结节,操作快捷、安全,定位准确,特别在直径≤10 mm的肺微小结节的定位中具有一定的优势。
Objective To observe the clinical value of artificial intelligence three-dimensional reconstruction technology in the preoperative positioning of small pulmonary nodules with single-port thoracoscopic lung segment resection.Methods The clinical data of 50 patients with small pulmonary nodules who underwent single-port thoracoscopic precision resection of lung segments in our hospital from January to June 2020 were selected.All use artificial intelligence three-dimensional reconstruction technology for positioning,analyze the surgical results,the success rate of preoperative positioning,the average operation time and the incidence of complications.Results Of the preoperative localization of 50 pulmonary nodules,there were 48 cases with successful localization and a success rate of 96.00%.All patients underwent single-port thoracoscopic precision resection of the lung segment within the same day.There were no cases of conversion to thoracotomy.All patients obtained clear pathological diagnosis results after surgery.The positioning operation time was 16-27 min,and the average positioning operation time was 18.23 min;the intraoperative blood loss was 53-66 ml,and the average blood loss was about 55.31 ml;the hospital stay was 8-25 d,and the average hospital stay was 15.21 d.There were 4 cases of asymptomatic pneumothorax,the incidence rate was 8.00%,and no complications such as hemoptysis and hemothorax occurred.Conclusion Using artificial intelligence three-dimensional reconstruction technology to locate small pulmonary nodules before the operation,the operation is fast,safe,and the positioning is accurate.Especially,it has certain advantages in the positioning of lung micro nodules with a diameter of≤10 mm.
