摘要
目的探讨颈部血管超声(CUS)与经颅多普勒超声(TCD)对高血压性脑出血患者预后评估作用。方法回顾性分析首次接受治疗的90例高血压性脑出血患者,患者出院3个月后对其进行随访,按预后情况不同分为预后良好组和预后不良组,预后良好组40例,预后不良组50例,分析颈部血管超声和经颅多普勒超声的检测参数与临床预后的关系。结果预后良好组椎动脉颅外段(VA)管径、阻力指数与预后不良组差异较大,具有统计学差异(P<0.05);预后良好组平均流速(VM)、舒张期最小流速(VD)、搏动指数(PI)与预后不良组差异较大,具有统计学差异(P<0.05);舒张期最小流速(VD)、搏动指数(PI)、阻力指数是高血压脑出血预后三个月不良的预测因素(P<0.05)。结论颈部血管超声与经颅多普勒超声诊断指标可为高血压性脑出血患者预后评估提供依据。
Objective To explore effects of carotid ultrasound(CUS)and transcranial Doppler ultrasound(TCD)on prognosis evaluation of patients with hypertensive cerebral hemorrhage(HCH).Methods A retrospective analysis was performed on 90 HCH patients who were treated for the first time in neurology department of the hospital.They were followed up after 3 months of discharge.According to different prognosis,they were divided into good prognosis group(40 cases)and poor prognosis group.good prognosis group(50 cases).The relationship between detection parameters of CUS and TCD and clinical prognosis was analyzed.Results There were significant differences in extracranial segmentdiameter of vertebral artery(VA)and resistance index between good prognosis group and poor prognosis group(P<0.05).There were significant differences inmean velocity(VM),diastolicvelocity(VD)and pulsatility index(PI)between good prognosis group and poor prognosis group(P<0.05).VD,PI and resistance index were poor predictive factors of prognosis after 3 months.Conclusion The diagnostic indexes of CUS and TCD can provide basis for prognosis evaluation of HCH patients.
