摘要
目的探讨经颅多普勒(TCD)超声联合颈部血管彩超在脑白质疏松症诊断中的临床意义。方法选取2017年8月至2019年1月本院收治的65例脑白质疏松症患者为研究组,选取同期与脑白质疏松症危险因素相匹配的无脑白质疏松症者65例为对照组。两组均进行TCD超声联合颈部血管彩超检查。结果研究组大脑中动脉收缩期血管流速、搏动指数均低于对照组(t=5.546、3.411,P=0.000),而颈内动脉斑块数量大于对照组(t=17.801,P=0.000);经Pearson相关性分析发现,斑块数量与收缩期血管流速呈负相关关系(r<0),而与搏动指数呈正相关关系(r>0)。结论TCD超声联合颈部血管彩超可有效评价脑白质疏松症患者血管情况,患者颈内动脉斑块数量较多,且大脑中动脉收缩期血液流速减缓,搏动指数值增高。
Objective To investigate the clinical significance of transcranial Doppler(TCD)ultrasound combined with cervical vascular color Doppler ultrasound in the diagnosis of leukopenia.Methods 65 patients with leukopenia admitted to our hospital from August 2017 to January 2019 were included in the study group.At the same time,65 patients with anencephaly who matched the risk factors of leukopenia were selected into the control group.Both groups underwent TCD ultrasound combined with cervical vascular color Doppler ultrasound examination.Results The systolic vascular flow rate and pulsatility index of the middle cerebral artery in the study group were lower than those in the control group(t=5.546,3.411,P=0.000),while the number of plaques in the internal carotid artery was greater than the control group(t=17.801,P=0.000);Pearson correlation analysis found that the number of plaques was negatively correlated with systolic vascular flow rate(r<0),and positively correlated with pulsatility index(r>0).Conclusion TCD ultrasound combined with cervical vascular color Doppler ultrasound can effectively evaluate the vascular condition of patients with leukopenia.There are more internal carotid artery plaques,and the blood flow rate of the middle cerebral artery slows down during systole,and the pulsatility index value increases.
