摘要
目的分析某院呼吸内科住院患者医院感染情况。方法对2015年1月-2018年12月某三甲医院呼吸内科所有住院患者进行回顾性调查。结果呼吸内科住院的6773例患者中,共发生医院感染210例,感染率为3.10%。不同年份的医院感染率呈下降趋势。医院感染患者的平均住院日数为22.73 d,主要感染部位排前5位的为下呼吸道、泌尿道、皮肤软组织、血流和上呼吸道。年龄、住院时间、侵入性操作与老年患者医院感染的发生有关(P<0.05)。分离出病原菌4365株,其中革兰阴性菌2996株,占68.64%;革兰阳性菌325株,占7.45%;真菌1044株,占23.92%。鲍曼不动杆菌对药敏试验的所有抗菌药物耐药率较高;铜绿假单胞菌对妥布霉素相对敏感;肺炎克雷伯菌对美罗培南、亚胺培南、头孢替坦和丁胺卡那霉素相对敏感;大肠埃希菌对亚胺培南、呋喃妥因、美罗培南和丁胺卡那霉素相对敏感。结论该院呼吸内科感染率高,检出病原菌主要以革兰阴性杆菌为主,鲍曼不动杆菌耐药率较高。
Objective To analyze the nosocomial infection of inpatients in respiratory medicine department of the hospital.Methods A retrospective survey were used for all inpatients in respiratory medicine department of a Grade-A tertiary hospital from January 2015 to December 2018.Results Among the 6773 patients hospitalized in respiratory medicine department 210 cases of nosocomial infection were found,with an infection rate of 3.10%.The rate of nosocomial infection in different years showed an overall downward trend.The average hospital stay of patients with nosocomial infection was 22.73 d.The top 5 main infection sites were lower respiratory tract,urinary tract,skin and soft tissue,bloodstream and upper respiratory tract.Age,length of stay,and invasive operation were related to the occurrence of nosocomial infections in elderly patients(P<0.05).Among 4365 strains of pathogenic bacteria isolated,2996 strains were Gram-negative bacteria,accounting for 68.64%;325 strains were Gram-positive bacteria,accounting for 7.45%;and 1044 strains were fungi,accounting for 23.92%.Acinetobacter baumannii was resistant to all antibiotics in drug sensitive test;Pseudomonas aeruginosa was relatively sensitive to tobramycin;Klesiella pneumoniae showed relatively sensitive to meropenem,imipenem,cefotetan and amikacin;Escherichia coli was relatively sensitive to imipenem,macrodantin,meropenem,amikacin.Conclusion Nosocomial infection rate in respiratory medicine department of this Grade-A tertiary hospital from January 2015 to December 2018 was high.The pathogenic bacteria isolated were mainly Gram-negative bacteria and Acinetobacter baumannii was the most resistant.
