摘要
目的明确神经外科有创机械通气患者呼吸机相关肺炎的感染途径及致病菌。方法选取某医院神经外科住院行有创机械通气且发生呼吸机相关肺炎的167例患者作为研究对象,根据呼吸机通气时间将患者分为早发型组和迟发型组,对患者致病菌及感染途径进行统计分析。结果迟发型组患者真菌菌株数明显多于早发型组患者且差异有统计学意义(p<0.05),其他致病菌两组患者间差异无统计学意义(p>0.05)。早发型组和迟发型组患者在人工气道套囊、呼吸机冷凝水、胃液、口腔及咽喉部分泌物等途径检出病原菌,其差异有统计学意义(p<0.01),而在呼吸机管路检出病原菌的差异无统计学意义(p>0.05)。结论相对于早发型呼吸机相关肺炎患者,迟发型呼吸机相关肺炎患者的真菌感染率更高,感染途径有一定差异,应引起重视。
Objective To clarify the infection route and pathogenic bacteria of ventilator-associated pneumonia in patients with invasive mechanical ventilation in neurosurgery.Methods 167 patients with ventilator-associated pneumonia who underwent invasive mechanical ventilation in a hospital neurosurgery department were selected as the research objects.According to the ventilator ventilation time,the patients were divided into early-onset group and late-onset group.Statistical analysis of pathogenic bacteria and infection routes was conducted.Results The number of fungal strains in the late-onset group was significantly higher than that in the early-onset group and the difference was statistically significant(p<0.05).There was no significant difference between the two groups of other pathogenic bacteria(p>0.05).Pathogenic bacteria were detected in early-onset group and late-onset group in artificial airway cuffs,ventilator condensate,gastric juice,oral and throat secretions,and the difference was statistically significant(p<0.01).There was no statistically significant difference in the detection of pathogenic bacteria in the pipeline(p>0.05).Conclusion Compared with patients with early-onset ventilator-associated pneumonia,patients with late-onset ventilator-associated pneumonia have a higher rate of fungal infection,and there are certain differences in the route of infection,which should be paid attention to.
