摘要
目的 了解2017年6月-12月河南省省立医院住院患者的呼吸道病原菌分布和药敏特征。方法 收集2017年6月-12月本院各病房送检的所有呼吸道标本,进行菌株的分离培养、鉴定及药敏实验,统计并分析呼吸道分离菌的分布及耐药性。结果 686份呼吸道标本共分离菌株235株,阳性率为34. 3%。其中肠杆菌68株（28. 9%）、非发酵菌58株（24. 7%）、葡萄球菌28株（11. 9%）、链球菌20株（8. 5%）、念珠菌61株（26. 0%）。多耐药菌株55株占比23. 4%,其中MRS 14株（6. 0%）、ESBLs 27株（11. 5%）、CRECO 4株（1. 7%）、CRAB 6株（2. 6%）、CRKPN 4株（1. 7%）。结论 呼吸道病原菌以革兰阴性杆菌为主,细菌耐药现象严重,特别是多耐药菌株比例较高,应引起医院和临床的高度重视。临床应重视患者药敏试验结果,合理使用抗菌药物,尽量减少经验性用药和过度治疗。
Objective To analyze the distribution and antimicrobial susceptibility of pathogens isolated from hospitalized patients with respiratory tract infection in Henan Provincial Hospital. Methods Respiratory specimens were collected from the wards during June -December in 2017, which were cultured, identified and conducted for antimicrobial susceptibility test, so as to analyze the distribution and antimicrobial susceptibility. Results A total of 235 strains were isolated from 686 respiratory specimens, and the positive rate was 34.3 %, including 68 strains of Enterobacteriaceae （28.9%）, 58 strains of Nonfermentaters （24.7 % ）, 28 strains of Staphylococcus （ 11.9% ）, 20 strains of Streptococcus （8.5 % ） and 61 strains of Candida （26.0%）. 55 strains of multi - drug resistant pathogens were detected, including 14 strains of MRS （6.0%）, 27 strains of ESBLs （ 11.5 % ）, 4 strains of CRECO （ 1.7% ）, 6 strains of CRAB （ 2.6% ）, 4 strains of CRKPN （ 1.7% ）. Conclusion Gram - negative bacteria was the primary pathogen, which was severely drug resistant. Multi - drug resistant strains occupied high proportion, which should be paid attention according to the results of susceptibility testing, so as to promote clinically rational drug use and minimize the use of empiric drugs and over - treatment.
