摘要
目的:研究我国冬季项目运动员前交叉韧带(ACL)损伤的临床表现特点,并提出了防治原则。方法:选取北京大学第三医院运动医学研究所2010年1月1日至2017年12月31日住院治疗的30例冬季项目运动员前交叉韧带损伤病例作为研究对象。查阅既往电子病历,记录研究对象的基本信息和病史,包括受伤情况、初次医疗处理情况和症状体征。查阅患者受伤后患膝MRI,评估患者受伤情况与合并损伤。查阅患者手术记录,结合术中关节镜录像评估患者合并损伤并确认手术方式。结果:30例前交叉韧带损伤病例,年龄19.76±4.06岁,其中女性病例20例。30例前交叉韧带损伤病例中有17例(56.7%)初诊诊断正确,另外13例并未诊断出ACL损伤。前交叉韧带损伤合并关节内其他结构损伤20例,占66.7%。前交叉韧带损伤合并外侧半月板损伤病例的手术间隔时间明显短于合并内侧半月板损伤的病例(P=0.032)。初次受伤到手术的时间间隔范围是8天~1000天,平均170.70±251.36天。30例前交叉韧带损伤病例术后均恢复原有运动水平,取得了良好的运动成绩。结论:冬季项目运动员前交叉韧带损伤多合并膝关节其它结构损伤,具有较高的危害性。需提高基层运动队队医对前交叉韧带损伤的诊疗水平,做到早诊断早治疗,使用微创手术的方法重建膝关节稳定性。
Objective To explore the clinical characteristics of anterior cmciate ligament injury in competitive winter sports athletes in China,and put forwards principles for prevention and treatment.Methods Thirty winter sports athletes who were admitted to Peking University Third Hospital in year2010 to 2017 for anterior cruciate ligament injury were selected as the research objects.Their electronic medical records were reviewed retrospectively to get the general condition and medical history,including injury,initial medical treatment,symptoms and signs.The MRI of the affected knees were also reviewed to evaluate their injuries.Moreover,their surgical records,combined with intraoperative arthroscopy video,were studied to evaluate their combined injuries and surgical methods.Results Among the 30 anterior cruciate ligament rupture patients with an average age of 19.76±4.06,20 were female,and 17(56.7%)were diagnosed correctly initially,but another 13 cases were not.Among all the patients,20 had combined structural injuries in the joint(66.7%).The operation interval of anterior cruciate ligament patients with lateral meniscus injury was significantly shorter than those with medial meniscus injury(P=0.032).The time gap between the injury and the operation ranged from 8 to1000 days,with an average of 170.70±251.36 days.All of the thirty cases recovered to their original sports level and achieved good results.Conclusion In winter sports athletes,their anterior cruciate ligament injury is often combined with other knee joint stmctural injuries,which is highly harmful.The ability of the first-line sports team doctors should be improved to achieve early diagnosis and treatment,and minimally invasive surgery should be conducted to reconstruct the stability of the knee joint.
