摘要
目的探讨前列腺穿刺术后间隔时间对根治性前列腺切除术后疗效及患者心理状态的影响。方法回顾性研究我院105例接受腹腔镜前列腺癌根治术患者围术期诊疗资料,手术间隔时间(IT)以4周为界将研究对象分成两组,收集其一般信息和围术期资料,包括:年龄,术前PSA,Gleason评分,穿刺针数,肿瘤分期,术后平均住院时间,手术时间,切缘阳性率,术中出血量,术后3、6、12个月尿控率及手术并发症发生率。通过综合性医院焦虑抑郁量表对行腹腔镜前列腺癌根治术两组患者穿刺后等待手术过程中的心理状态行量化评估。结果两组患者基础资料,手术时长,术后3、6、12个月尿控率,术后住院时间,切缘阳性率,术中失血量及手术并发症发生率比较差异无统计学意义(P>0.05)。对两组105例患者在等待手术过程中的心理状态量化评估提示,抑郁量表中两组患者平均分差异无统计学意义[(8.75±5.39)vs.(9.01±4.67),P=0.797],IT≥4周组在焦虑量表中平均分高于IT<4周组,且差异有统计学意义[(8.96±4.73)vs.(14.08±3.34),P<0.001]。结论针对腹腔镜前列腺癌根治术疗效及难易程度而言,间隔时间虽不能作为其影响要素,但间隔时间延长促进了患者心理的焦虑状态。
Objective To explore the effect of interval time(IT)between prostate biopsy and radical prostatectomy on the clinical efficacy and psychological status of patients.Methods The clinical data of 105 patients undergoing laparoscopic radical prostatectomy were retrospectively analyzed.According to the length of IT(≥4 weeks and<4 weeks),the patients were divided into two groups.Perioperative and general information of patients were collected,including age,preoperative PSA,Gleason score,number of puncture needles,tumor staging,intraoperative blood loss,operation time,positive margin rate,average postoperative hospital stay,urine control 3,6 and 12 months after operation,and complications.The psychological status of patients waiting for surgery was quantitatively assessed with anxiety and depression scale.Results There were no statistical differences in the operation time,urine control 3,6 and 12 months after operation,intraoperative blood loss,positive margin rate and complications between the two groups(P>0.05).There was no difference in the depression score between the two groups[(8.75±5.39)vs.(9.01±4.67),P=0.797],but IT≥4 group had higher anxiety score than IT<4 group[(8.96±4.73)vs.(14.08±3.34),P<0.001].Conclusion The IT between prostate biopsy and radical prostatectomy does not affect the surgical difficulty or effect of surgery,but prolonged IT can aggravate the psychological anxiety of patients.
