2017-2019年江苏省输入性D8和B3基因型麻疹的分子流行病学特征 被引量:2

Molecular epidemiology of imported D8 and B3 genotype measles in Jiangsu province,2017-2019

摘要 目的分析江苏省输入性D8基因型、B3基因型麻疹病毒(Measles virus,MV)病例分子流行病学特征。方法对2017-2019年江苏省疑似麻疹病例开展流行病学调查,采集咽拭子标本进行MV核酸检测、分离培养、N基因RT-PCR扩增和序列测定,分析MV分离株与GenBank中24个基因型参考株的核苷酸和氨基酸同源性,描述输入性MV病例特征。结果从2017-2019年麻疹报告病例中获得12株MV,其中H1a、D8、B3基因型分别为8株、3株、1株。H1a基因型MV间核苷酸、氨基酸同源性均为100%;D8基因型MV与澳大利亚、美国、日本、文莱、阿根廷MV株核苷酸、氨基酸同源性分别为98.7%-100%、97.3%-100%;B3基因型MV与美国、澳大利亚MV株核苷酸、氨基酸同源性均为100%。3例D8基因型病例含麻疹成分疫苗(MCV)免疫史不详,其中1例有埃及旅行史;1例B3基因型病例无MCV免疫史,有菲律宾旅行史和麻疹确诊病例接触史。结论2017-2019年江苏省麻疹流行仍以本土H1a基因型MV为主,并出现输入性D8和B3基因型MV;需加强麻疹分子流行病学监测。 Objective To analyze the molecular epidemiology of imported genotype D8 and B3 measles viruses(MV)in Jiangsu province.Methods We conducted an epidemiological survey of suspected measles cases during 2017-2019 in Jiangsu and collected throat swabs for MV nucleic acid detection,MV isolation,and RT-PCR amplification and sequencing of MV N genes.We analyzed nucleotide and amino acid homologies between MV isolates and 24 MV genotype reference strains from GenBank and described characteristics of import-related measles cases.Results A total of 12 MV strains were obtained from measles cases reported during 2017-2019,among which eight,three,and one strains were H1 a,D8,and B3 genotypes.The nucleotide and amino acid homologies were 100%within H1 a genotype strains;98.7%-100%and 97.3%-100%between D8 genotype strains and reference strains from Australia,the United States,Japan,Brunei and Argentina;and 100%between the B3 genotype strain and reference strains from the United States and Australia.Measles vaccination histories were unknown for three D8 genotype cases,one of which had traveled to Egypt.One B3 genotype case had not received measles containing vaccine and had traveled to the Philippines and had contacted with a confirmed measles case.Conclusions The predominant MV strain circulating in Jiangsu was the indigenous H1 a genotype;D8 and B3 genotype measles viruses were imported.We should strengthen molecular epidemiological surveillance of measles.
