摘要
目的探究高频超声对甲状腺微小癌的诊断价值,并分析其超声图像特点。方法以2017年1月至2019年1月接收的30例疑似甲状腺微小癌患者为研究对象。所有患者均接受高频超声和病理检查。分析其检查的结果。结果高频超声对甲状腺微小癌的检出率为91.67%,疾病漏诊率为8.33%。高频超声诊断甲状腺微小癌的灵敏度为91.67%,特异度为100.00%,准确度为93.33%。高频超声检出22例甲状腺微小癌,其中21例(95.45%)为低回声,21例(95.45%)为实性病灶,20例(90.91%)病灶边界模糊,22例(73.33%)病灶形态不规则,14例(63.64%)病灶纵横比≥1,14例(63.64%)存在微小钙化,18例(81.82%)病灶后方回声无变化,12例(54.55%)病灶与甲状腺被膜清晰,8例(36.36%)血流分级为1级,7例(31.82%)血流分级为2级,7例(31.82%)血流分级为3级。结论应用高频超声检查甲状腺微小癌患者的临床价值较高,且可反映患者的具体疾病情况,为患者疾病的确诊以及治疗方案的制定提供参考依据。
Objective To explore the diagnostic value of high frequency ultrasound for thyroid microcarcinoma,and to analyze the characteristics of ultrasound images.Methods Thirty patients with suspected thyroid microcarcinoma enrolled from January 2017 to January 2019 were selected as the research objects.All patients received high frequency ultrasound and pathological examination,and the examination results were analyzed.Results The detection rate of thyroid microcarcinoma by high frequency ultrasound was 91.67%,and the missed diagnosis rate was 8.33%.The sensitivity,specificity and accuracy of high frequency ultrasound in the diagnosis of thyroid microcarcinoma were 91.67%,100.00%and 93.33%,respectively.High frequency ultrasound detected 22 cases of thyroid microcarcinoma,of which 21 cases(95.45%)were hypoechoic,21 cases(95.45%)were solid lesions,20 cases(90.91%)had blurred borders,22 cases(73.33%)had irregular lesion morphology,14 cases(63.64%)had lesions aspect ratio≥1,14 cases(63.64%)had microcalcifications,18 cases(81.82%)had no change in echoes behind the lesions,12 cases(54.55%)had clear lesions and thyroid capsule,8 cases(36.36%)had grade 1 blood flow classification,7 cases(31.82%)had grade 2 blood flow classification,and 7 cases(31.82%)had grade 3 blood flow classification.Conclusion The high frequency ultrasound in the diagnosis of patients with thyroid microcarcinoma has high clinical application value,which can reflect the specific disease situation of patients,and provide reference for the diagnosis of disease and the formulation of treatment plan.
