摘要
目的研究探讨中医护理在老年股骨粗隆间骨折患者术后疼痛与髋关节活动度的应用效果。方法选取2019年6月-2019年12月收治的老年股骨粗隆间骨折患者98例为研究对象,随机分为观察组、对照组,各49例。两组患者均给予防旋股骨髓内钉(PFNA)内固定手术治疗,对照组在治疗期间给予常规护理干预,观察组在手术期间给予中医护理干预。对两组患者术后的并发症发生率、疼痛评分、髋关节活动度、生活质量评分、护理满意度进行统计对比。结果观察组并发症发生率为8.16%(4/49),低于对照组患者为24.49%(12/49),组间对比有显著性差异(P<0.05)。患者在术后1周、4周、12周的视觉模拟评分(VAS)均低于对照组(P<0.05)。患者在术后1周、4周、12周的Harris髋关节功能评分均高于对照组(P<0.05)。术后3月时观察组生活质量评分均高于对照组(P<0.05)。观察组总护理满意度为91.86%(45/49),高于对照组患者为73.47%(36/49),组间对比有显著性差异(P<0.05)。结论中医护理在老年股骨粗隆间骨折患者术后护理效果良好,能显著降低疼痛感,提高髋关节活动度,降低术后并发症发生率,提高术后的生活质量,值得推荐。
Objective To study the effect of traditional Chinese medicine nursing on postoperative pain and hip joint mobility in elderly patients with intertrochanteric fracture of femur.Methods From June 2019 to December 2019,98 elderly patients with intertrochanteric fracture of femur were selected as the study objects,and were randomly divided into the observation group and the control group,49 cases in each.The patients in the two groups were treated with internal fixation of PFNA.The patients in the control group were given routine nursing intervention during the treatment,and those in the observation group were given traditional Chinese medicine nursing intervention during the operation.The incidence of postoperative complications,pain score,hip joint activity,quality of life score and nursing satisfaction were compared between the two groups.Results The incidence of complications in the observation group was 8.16%(4/49),which was lower than that in the control group24.49%(12/49).There was significant difference between the two groups(P<0.05).The visual analogue scores(VAS)of the patients at 1,4 and 12 weeks after operation were lower than those of the control group(P<0.05).The Harris hip joint function scores of the patients at 1,4 and 12 weeks after operation were higher than those of the control group(P<0.05).The scores of quality of life in the observation group were higher than those of the control group at 3 months after operation(P<0.05).The total nursing satisfaction of the observation group was 91.86%(45/49),which was higher than that of the control group(73.47%,36/49).Conclusion Traditional Chinese medicine nursing in the elderly patients with intertrochanteric fracture postoperative care has goodeffect and can significantly reduce the pain of patients,improve the hip joint activity of patients,reduce the incidence of postoperative complications,improve the quality of life of patients after surgery.It is worthy of clinical recommendation.
