摘要
目的探讨宫颈癌前病变筛查中宫颈液基薄层细胞学检测(TCT)与阴道镜联合检查的效果比较。方法方便选择该院2015年1月—2019年12月妇科门诊接待的宫颈癌前病变筛查患者491例进行研究,入组对象自愿配合研究,均接受TCT、阴道镜及二者联合筛查。同时,均于阴道镜下多位点活检,将病理学检查结果作为标准,比较TCT、阴道镜、二者联合筛查的效果。结果 491例宫颈癌前病变筛查患者经TCT检查,检出异常59例,阴道镜检查检出异常82例,TCT联合阴道镜检查检出异常89例;TCT联合阴道镜筛查特异性为95.90%;敏感性为94.74%、漏诊率为0.81%;TCT筛查特异性为96.39%、敏感性为57.89%、漏诊率为6.52%,阴道镜筛查特异性为97.35%、敏感性为93.42%、漏诊率为1.02%;统计学分析,显示TCT联合阴道镜筛查在敏感性与漏诊率上明显优于TCT筛查(P<0.05)。结论 TCT与阴道镜检查在宫颈癌前病变筛查中均有不错的价值,但二者联合筛查可提高宫颈癌前病变检出率,尽早检出异常情况,便于早期诊断与治疗,从而为临床提供更多可靠的依据。
Objective To explore the comparison of the effects of cervical liquid-based thin-layer cytology(TCT) and colposcopy combined in the screening of cervical precancerous lesions. Methods The Convenient study selected 491 cervical precancerous lesion screening patients received in this hospital from January 2015 to December 2019 for research.The enrolled subjects voluntarily cooperated in the study, and all received TCT, colposcopy and combined screening of the two. At the same time, multi-site biopsy was performed under colposcopy, and pathological examination results were used as the standard to compare the effects of TCT, colposcopy, and the combined screening of the two. Results 491 patients with cervical precancerous lesions were screened by TCT, 59 were abnormal, 82 were abnormal by colposcopy, and 89 were abnormal by TCT combined with colposcopy;the specificity of TCT combined with colposcopy was as follows: 95.90%,sensitivity was 94.74%, missed diagnosis rate was 0.81%. TCT screening specificity was 96.39%, sensitivity was 57.89%,missed diagnosis rate was 6.52%. colposcopy screening specificity was 97.35%, sensitivity was 93.42%, the missed diagnosis rate was 1.02%. Statistical analysis showed that TCT combined with colposcopy screening was significantly better than TCT screening in sensitivity and missed diagnosis rate(P<0.05). Conclusion TCT and colposcopy have good value in the screening of cervical precancerous lesions, but the combined screening of the two can increase the detection rate of cervical precancerous lesions, detect abnormalities as soon as possible, and facilitate early diagnosis and treatment, to provide more reliable basis for the clinic, it is worth applying.
