检验科微生物检验质量的影响因素与病原菌耐药性研究 被引量:4

摘要 目的探究检验科微生物检验质量的相关影响因素和病原菌耐药性。方法于本院2017年5月至2018年5月,随机抽取618例感染性疾病患者标本,对检验人员的操作过程全程监视与准确记录,对检验过程中采用的设备、应用时间等情况准确记录,对影响微生物检验质量的相关因素有效调查。结果本研究误差标本118例,误差率19.09%。实验室检测标本类型有血常规、尿常规、病原菌培养脑脊液、蛋白定量、其他,准确率分别为78.03%、86.13%、76.09%、81.82%、82.09%;影响因素主要有实验室人员素质以及采集标本质量等。常见致病菌有革兰阴性菌320例(51.78%),革兰阳性菌244例(39.48%),真菌54例(8.74%),其中真菌所占比例低于革兰阴性菌与革兰阳性菌,差异有统计学意义(P<0.05);金黄色葡萄球菌在红霉素类、β-内酰胺类以及喹诺酮类药物的耐药率方面均大于50.00%;大肠埃希菌在头孢类以及喹诺酮类药物耐药率方面均大于50.00%。结论对检验科微生物检验质量影响因素充分了解,并制定相应的干预措施,可使检验效果显著提高,借助耐药性实验分析可为临床合理选择治疗药物起到促进作用。 Objective To investigate the related factors and pathogen resistance of microorganism inspection quality in the department of laboratory.Methods During the period from May 2017 to May 2018,618 specimens of infectious diseases were randomly selected,and the inspection personnel were monitored and accurately recorded throughout the operation process,and the equipment used in the inspection process and the application time were accurately recorded.To investigate the related factors that affect the quality of microorganism inspection effectively.Results One hundred and eighteen cases of error specimens with an error rate of 19.09%.The types of specimens tested in the laboratory included blood routine,urine routine,pathogenic bacteria culture cerebrospinal fluid,protein quantification,other 173 cases,173 cases,92 cases,77 cases,67 cases,36 cases,and the exact number of cases was 135 cases,149 cases,70 cases,63 cases,55 cases,and 28 cases,respectively.The accuracy rates were 78.03%,86.13%,76.09%,81.82%,82.09%,respectively;The influencing factors mainly include the quality of laboratory personnel and the quality of collected specimens.Common pathogenic bacteria are gramnegative bacteria 51.78%,gram-positive bacteria 39.48%,and fungi 8.74%.The proportion of fungi is lower than that of gram-positive bacteria,and the difference is statistically significant(P<0.05);staphylococcus aureus has a tolerance of more than 50.00%in erythromycin,β-lactam,and quinolones.Escherichia coli is more than 50.00%resistant to cefres and quinolones.Conclusion To fully understand the factors that affect the quality of microorganism inspection in the laboratory,and formulate the corresponding intervention measures,can significantly improve the inspection effect,and the drug resistance experimental analysis can promote the rational choice of therapeutic drugs for clinical purposes.
