川崎病患儿急性期中性粒细胞CD64、降钙素原及IL-38水平变化及其临床意义 被引量:2

The Changes of CD64,Procalcitonin and IL-38 Levels of Neutrophils in Children with Kawasaki Disease and Their Clinical Significance

摘要 目的:探讨川崎病患儿急性期中性粒细胞CD64、降钙素原(PCT)及白细胞介素-38(IL-38)水平变化及其临床意义。方法:选取笔者所在医院2014年10月-2019年9月收治的80例川崎病患儿为观察组。根据患儿有无冠脉损伤分为冠脉损伤组(36例)和无冠脉损伤组(44例),另选择同期健康体检患儿40例为对照组。比较各组血清CD64、PCT、IL-38水平,观察组治疗前后血清CD64、PCT、IL-38水平,血清CD64、PCT、IL-38与冠脉损伤程度的相关性。结果:冠脉损伤组血清CD64高于无冠脉损伤组,IL-38低于无冠脉损伤组,差异均有统计学意义(P<0.05),两组PCT比较差异无统计学意义(P>0.05);冠脉损伤组及无冠脉损伤组CD64、PCT均高于对照组,IL-38均低于对照组,差异均有统计学意义(P<0.05);观察组患儿治疗后CD64、PCT均低于治疗前,IL-38高于治疗前,差异均有统计学意义(P<0.05);CD64与冠脉损伤程度呈正相关性(P<0.05),IL-38与冠脉损伤程度呈负相关性(P<0.05),PCT与冠脉损伤程度无明显相关性(P>0.05)。结论:CD64、PCT、IL-38均可作为川崎病患儿急性期的敏感因子,为该病的临床抗菌抗炎治疗提供一定理论指导,有利于改善预后。 Objective:To investigate the changes and clinical significance of CD64,procalcitonin(PCT)and IL-38 in children with Kawasaki disease.Method:A total of 80 children with Kawasaki disease admitted to our hospital from October 2014 to September 2019 were selected as the observation group.According to the presence or absence of coronary artery injury,the children were divided into the coronary artery injury group(36 cases)and the non-coronary artery injury group(44 cases),in addition,40 healthy children were selected as the control group.The levels of serum CD64,PCT and IL-38 were compared before and after treatment,and the correlation between the levels of serum CD64,PCT and il-38 and the degree of coronary injury were observed.Result:The levels of CD64 and PCT in the coronary artery injury group were higher than those in the the non-coronary artery injury group,the level of IL-38 in the coronary artery injury group was lower than those in the non-coronary artery injury group,the differences were statistically significant(P<0.05).The levels of CD64 and PCT in the coronary artery injury group and the non-coronary artery injury group were higher than those in the control group,and the levels of IL-38 were lower than those in the control group,the differences were statistically significant(P<0.05).After treatment,CD64 and PCT of the children in the observation group were lower than those before treatment,while IL-38 was higher than that before treatment,the differences were statistically significant(P<0.05).CD64 was positively correlated with the degree of coronary injury(P<0.05),IL-38 was negatively correlated with the degree of coronary artery injury(P<0.05),PCT had no significant correlation with the degree of coronary artery injury(P>0.05).Conclusion:CD64,PCT and IL-38 can be used as sensitive factors in the acute phase of children with Kawasaki disease,which can provide theoretical guidance for the clinical anti-bacterial and anti-inflammatory treatment of the disease and improve the prognosis.
