摘要
目的探究血清转化生长因子-β1(TGF-β1)、白细胞介素-34(IL-34)、核转录因子κB(NF-κB)在IgA肾病患者中的表达及其与纤维化的关系。方法选取2017年1月到2019年11月我院收治的108例IgA肾病患者,按照肾间质纤维化分级标准将患者分为轻度组(n=42,T0级),中度组(n=36,T1级)和重度组(n=30,T2级),另选我院体检健康人员32例作为对照组,分析血清TGF-β1、IL-34、NF-κB在IgA肾病患者中的表达及其与纤维化的关系。结果IgA肾病患者各组24 h尿蛋白、血肌酐均高于对照组,eGFR、血总蛋白低于对照组,且与轻度组相比,中度组、重度组24h尿蛋白、血肌酐水平逐渐升高,eGFR、血总蛋白逐渐降低,差异具有统计学意义(P<0.05);IgA肾病患者各组血清TGF-β1、IL-34、NF-κB水平高于对照组,且与轻度组相比,中度组、重度组血清TGF-β1、IL-34、NF-κB水平逐渐升高,差异具有统计学意义(P<0.05);Pearson相关性分析显示,血清TGF-β1、IL-34、NF-κB与24h尿蛋白、血肌酐均呈正相关(r>0,P<0.05),与eGFR、总蛋白呈负相关(r<0,P<0.05);多元线性回归分析。结果显示IgA肾病患者肾间质纤维化程度与血清TGF-β1、IL-34、NF-κB水平呈正相关(P<0.05)。结论血清TGF-β1、IL-34、NF-κB在IgA肾病患者中高表达,且与纤维化程度呈正相关,能在一定程度上反映病情严重程度。
Objective To investigate the expressions of serum transforming growth factor-β1(TGF-β1),interleukin-34(IL-34),and nucleus transcription factorκB(NF-κB)in patients with IgA nephropathy and their correlations with Fibrotic.Methods A total of 108 patients with IgA nephropathy admitted to our hospital from January,2017 to November,2019 were enrolled and divided into mild(n=42,T0),moderate(n=36,T1 level)and severe groups(n=30,T2 level),and another 32 healthy people from our hospital were selected as controls.The expressions of TGF-β1,IL-34 and NF-κB in patients with IgA nephropathy and their relationships with fibrosis were then analyzed.Results The 24h urine protein and serum creatinine in IgA nephropathy patient groups were higher than those in the control group;in addition,eGFR and total blood protein were lower than those in the control group.Compared with the mild group,the 24h urine protein,and serum creatinine levels in the moderate group and severe group gradually increased,while eGFR and total blood protein gradually decreased(P<0.05).Serum levels of TGF-β1,IL-34 and NF-κB in IgA nephropathy patient groups were higher than those in the control group.Compared with the mild group,the serum levels of TGF-β1,IL-34 and NF-κB in the moderate and severe groups increased gradually(P<0.05).Pearson correlation analysis showed that serum TGF-β1,IL-34 and NF-κB were positively correlated with 24h urine protein and serum creatinine(r>0,P<0.05),while negatively correlated with eGFR and total protein(r<0,P<0.05).Multiple linear regression analysis showed that the degree of renal interstitial fibrosis in patients with IgA nephropathy was positively correlated with serum levels of TGF-β1,IL-34,and NF-κB(P<0.05).Conclusion Serum TGF-β1,IL-34 and NF-κB are highly expressed in patients with IgA nephropathy,and positively correlated with the degree of fibrosis,which can reflect the disease severity to a certain extent.
