慢性肾病各期中血清NGAL、CysC和肾功能指标的变化及其临床意义 被引量:3

摘要 目的:探究慢性肾病(CKD)各期中性粒细胞明胶酶相关载脂蛋白(NGAL)、胱抑素C(CysC)和肾功能指标[肌酐(SCr)、尿素(UREA)和尿酸(UA)]的表达变化及其临床意义。方法:选择2017年9月-2019年9月106例来我院就诊并确诊为慢性肾病患者为研究对象,按2013年美国国家肾脏基金会(NKF)肾脏病预后质量指南(K/DOQI)的CKD诊断标准分为:CKD1期18例、CKD2期21例、CKD3期23例、CKD4期16例、CKD5期28例。随机选择同时期我院体检的人群100例作为对照组,分析各组人群血清中NGAL、CysC、SCr、UA、UREA的含量。分析血清中NGAL水平与CysC以及肾功能指标之间的相关性。结果:与健康对照组相比,CKD各期的血清NGAL、CysC、UA、UREA水平均显著升高(P<0.05)。CKD3、4期SCr水平与健康对照相比均显著升高(P<0.05);而CKD1、2期SCr和健康对照组比,差异不具有统计学意义(P>0.05)。Pearson相关分析发现,血清NGAL与肾功能各项指标(SCr、UA、UREA)、CysC呈现正相关(P<0.05),且与GFR存在负相关性(P<0.05)。结论:NGAL可以作为CKD患者早期肾功能损害的一个既敏感又特异的生物学标志物。在一定程度上,血清NGAL的水平能代替GFR反映肾脏受损的严重性。 Objective:To investigate the expression and clinical significance of neutrophil gelatinase associated lipocalin,Cystatin C and renal function(including Scr,UREA and UA)in patients with chronic kidney disease.Methods:In our hospital,106 patients with CKD from September 2017 to September 2019,according to the criteria of CKD stage formulated by KDOQI,the standard of CKD was divided into 18 cases of CKD1 stage,21 cases of CKD2 stage,23 cases of CKD3 period,16 cases of CKD4 period and 28 cases of CKD5 stage.100 people with physical examination in the same period were selected as the control group.The concentration of NGAL,CysC,SCr,UA,UREA in serum of each group was analyzed.The correlation between NGAL level and CysC and renal function indexes was analyzed.Results:Compared with the control group,the levels of serum NGAL,CysC,UA,UREA in each stage of CKD increased significantly(P<0.05).The SCr levels of CKD3 and CKD4 increased significantly than in healthy controls(P<0.05),however,the difference SCr between CKD stage 1,2 and healthy controls was not statistically significant(P>0.05).Pearson correlation analysis showed that NGAL was positively correlated with renal function indexes(SCr,UA,UREA)and cystine C(P<0.05).There was a negative correlation between NGAL and GFR(P<0.05).Conclusion:NGAL can be used as a sensitive and specific biomarker for early renal function damage in CKD patients.To some extent,the level of serum NGAL can replace GFR to reflect the severity of kidney damage.
