慢性肾病患者蛋白尿程度与血NGAL、胱抑素C的相关性分析 被引量:3

摘要 【目的14g讨慢性肾脏病(CKD)患者血中性粒细胞明胶酶相关脂质运载蛋白(NGAL)、胱抑素c(Cysc)的改变情况及其临床意义。【方法】以2014年3月至2015年9月在本院治疗的120例CKD患者为观察对象。根据尿微量白蛋白/肌酐比值(ACR)分为正常白蛋白尿组(NA组,38例)、微量白蛋白尿组(MA组,42例)和临床白蛋白尿组(CA组,40例),同时选取40例健康成年人作为对照组。观察各组NGAL、CysC水平;比较各组肾功能指标和铁代谢相关指标的差异,分析NGAL、CysC与肾功能指标和铁代谢相关指标的相关性。【结果】四组的NGAL、CysC水平为:CA纽〉MA纽〉NA组〉对照组(P〈0.05);尿素氮(BUN)、血尿酸(SUA)和血肌酐(SCr)水平为:CA组〉MA组〉NA组〉对照组(P〈0.05);四组红细胞(RBC)、血红蛋白(Hb)、红细胞比容(Hct)和铁蛋白(SF)水平为:cA组〈MA组〈NA组〈对照组(P〈0.05);NGAL、CysC与BUN等肾功能指标显著正相关,与SF等铁代谢相关指标显著负相关(P〈0.05)。【结论ICKD患者血NGAL、CysC水平明显高于对照组,且CysC以及NGAL对于评估CKD病情变化具有重要意义,其水平与患者肾功能指标密切相关。 [Objective]To observe the levels of neutrophil gelatinase-associated protein (NGAL) and cystatin C (Cys C) in patients with chronic kidney disease (CKD) and analyze its clinical significance. [Methods]The 120 pa- tients with CKD who were treated in our hospital from March, 2014 to September, 2015 were observed.According to urine albumin/creatinine ratio (ACR), patients were divided into normal albuminuria group (NA group, 38 ca- ses), microalbuminuria group (MA group, 42 cases), and clinical albumin urinary group (CA Group, 40 cases). Meanwhile 40 cases of healthy adults were selected as the control group. The levels of blood NGAL and cystatin C in the different groups were detected, the renal function parameters and iron metabolism related indicators were compared, furthermore,the correlations of NGAL and cystatin C with renal function indices and iron metabolism related indicators were analyzed. [Results]The levels of NGAL and cystatin C in the four groups were: CA Group 〉MA group 〉NA group 〉 control group ( P 〈0.05). The level of BUN, blood urea, and SCr renal function in- dices in the four groups were: group CA〉group MA〉group NA 〉control group ( P 〈0.05). The levels of RBC, Hgb, Hct, and SF in the four groups were: CA group 〈NA group〈MA 〈 control group ( P 〈0.05). There was positive correlation among NGAL, Cystatin C, and renal function indicators like BUN, while there was significant negative correlation among NGAL, Cystatin C, and iron metabolism as SF ( P 〈0.05). [Conclusion] The levels of NGAL and Cystatin C in patients with CKD are significantly higher than the control group, and their levels are closely related to renal function indices.
