摘要
目的:系统评价华蟾素胶囊联合化疗治疗晚期结直肠癌的疗效和安全性。方法:检索中国期刊全文数据库(CNKI)、万方数据库(WanFang)、中国生物医学文献数据库(CBM)、维普资讯中文科技期刊数据库(VIP)、Medline、Cochrane Library、Web of Science,收集华蟾素胶囊治疗晚期结直肠癌的随机对照试验(randomized controlled trials,RCTs),检索时间为该数据库建库至2019年10月。采用RevMan 5.3对纳入研究的近期有效率、生活质量稳定率、疼痛缓解率及不良反应进行Meta分析,结局效应指标采用相对危险度(relative risk,RR),以其95%可信区间(confidence interval,CI)表示。结果:共纳入9项RCTs涉及658例结直肠癌患者,试验组均为华蟾素胶囊联合化疗药物,对照组为单用化疗药物。Meta分析结果显示,华蟾素胶囊联合化疗药物在近期有效率、患者生活质量、疼痛缓解率方面明显优于单纯使用化疗药物,并可降低白细胞毒性、胃肠不良反应、肝功能异常、手足麻木的发生率。结论:在化疗药的基础上加用华蟾素胶囊可在提高近期有效率、改善患者生活质量及提高疼痛缓解率的同时降低白细胞毒性、胃肠道不良反应、肝功能损害及手足麻木的发生,但由于纳入临床研究质量整体偏低,故仍需大样本、多中心、高质量的RCTs进行验证。
Objective:To systematically evaluate the efficacy and safety of Cinobufacini capsule combined with chemotherapy in the treatment of advanced colorectal cancer.Methods:Databases of Chinese National Knowledge Infrastructure(CNKI),WanFang data(WanFang),VIP Database for Chinese Technical Periodicals(VIP),China Biology Medicine(CBM),Medline,Cochrane library and Web of Science were searched for randomized controlled trials(RCT S)on Cinobufacini capsule in treating advanced colorectal cancer from the database establishment to October 2019.Effective rate,quality of life,pain relief rate and adverse reactions were analyzed by RenMan5.3 software.Outcomes were shown as Relative Risk(RR)and its 95%Confidence Interval(95%CI).Results:Nine RCTs involving 658 patients were included totally.Meta-analysis showed that Cinobufacini capsule combined with chemotherapy superior to the chemotherapy at the aspects of the effective rate,quality of life,pain relief rate and and inferior to the chemotherapy in the leukopenia,gastrointestinal adverse reaction,liver dysfunction and numbness of hands and feet.Conclusion:Cinobufacini capsule combined with chemotherapy could improve the effective rate and pain relief rate,improve the quality of life for patients and reduce the incidence of adverse reactions such as leukopenia,gastrointestinal response,liver function damage and numbness of hands and feet.However,this study is only a preliminary evaluation,which needs to be verified by large sample,multicenter,high quality RCTs.
