摘要
目的通过检测冠心病(coronary heart disease,CHD)患者血清脂肪特异性丝氨酸蛋白酶抑制剂(Vaspin)、Apelin、Chemerin水平,探讨三者与CHD患者血压变异性及Gensini积分的相关性。方法选取行冠状动脉造影确诊为CHD患者122例为冠心病组,另选取同期于本院住院的非冠CHD患者122例作为对照组。采用酶联免疫吸附法(ELISA)检测血清Vaspin、Apelin、Chemerin水平。采用Gensini积分对CHD患者病变程度进行评分评估。对所有受检者进行24 h动态血压监测,记录分析血压变异性各项指标,包括:24 h收缩压标准差(24hSSD)、24 h舒张压标准差(24hDSD)、白昼收缩压标准差(dSSD)、白昼舒张压标准差(dDSD)和夜间收缩压标准差(nSSD)、夜间舒张压标准差(nDSD)。采用Pearson法分析Vaspin、Apelin、Chemerin水平与CHD患者血压变异性及Gensini积分的关系。结果冠心病组Gensini积分为(36.27±12.49)分;冠心病组血清Vaspin、Apelin水平均显著低于对照组,Chemerin水平显著高于对照组,差异有统计学意义(P<0.05);冠心病组24hSSD、24hDSD、dSSD、dDSD、nSSD、nDSD均高于对照组,差异有统计学意义(P<0.05)。CHD患者血清Vaspin、Apelin水平与Gensini积分呈负相关,Chemerin水平与Gensini积分呈正相关(P<0.05)。血清中Vaspin水平均与24hSSD、24hDSD、dSSD、dDSD、nSSD、nDSD均呈负相关(P<0.05);Chemerin水平与24hSSD、24hDSD、dSSD、dDSD、nSSD、nDSD均呈正相关(P<0.05);Apelin水平与24hSSD、24hDSD、dSSD、nSSD、nDSD均呈负相关(P<0.05)。结论CHD患者血清Vaspin、Apelin水平明显降低,Chemerin水平显著升高,可能与CHD的血压变异性及Gensini积分有关。
Objective To investigate the correlations between levels of serum fat-specific serine protease inhibitor(Vaspin),Apelin,Chemerin and blood pressure variability,Gensini score in patients with coronary heart disease(CHD).Methods A total of 122 patients with CHD diagnosed by coronary angiography were selected as the patients group,and 122 patients without CHD who underwent physical examination during the same period were selected as the control group.The levels of Vaspin,Apelin and Chemerin in serum were detected by enzyme-linked immunosorbent assay(ELISA).Gensini score was used to evaluate the degree of pathological changes in CHD patients.The 24-hour ambulatory blood pressure monitoring was performed on all subjects,and variability of blood pressure was recorded and analyzed,including 24-hour standard deviation of systolic pressure(24 hSSD),24-hour standard deviation of diastolic pressure(24 hDSD),day standard deviation of systolic pressure(dSSD),day standard deviation of diastolic pressure(dDSD),night standard deviation of systolic pressure(nSSD),and night standard deviation of diastolic pressure(nDSD).Pearson method was used to analyze the correlations between Vaspin,Apelin and Chemerin levels and blood pressure variability,Gensini score in CHD patients.Results Gensini score was 36.27±12.49 in the patient group,the levels of serum Vaspin and Apelin in patients were significantly lower than those in the control group,and the level of chemerin was significantly higher than that in the control group(P<0.05).The 24 hSSD,24 hDSD,dSSD,dDSD,nSSD and nDSD in the patients group were significantly higher than those in the control group(P<0.05).Serum Vaspin and Apelin levels were significantly negatively correlated with Gensini score,while Chemerin level was significantly positively correlated with Gensini score(P<0.05).The level of serum Vaspin was significantly negatively correlated with 24 hSSD,24 hDSD,dSSD,dDSD,nSSD and nDSD(P<0.05).Chemerin level was significantly negatively correlated with 24 hSSD,24 hDSD,dSSD,dDSD,
