摘要
目的探究食管鳞癌组织中血管内皮生长因子(VEGF)、缺氧诱导因子1α(HIF-1α)表达对食管鳞癌患者放疗疗效的影响。方法以80例食管鳞癌患者为研究对象,所有患者均接受放疗治疗。以其活检病理癌组织标本作为观察组(n=80),癌旁组织标本作为对照组(n=80)。比较癌组织及癌旁组织中VEGF、HIF-1α表达情况,并分析VEGF、HIF-1α表达的相关性。对可能影响食管鳞癌患者放疗近期疗效的因素进行单因素及多因素分析。结果VEGF、HIF-1α在观察组中的阳性表达率均显著高于对照组(67.5%vs.7.5%,76.25%vs.11.25%,P<0.05);VEGF和HIF-1α的表达呈正相关。放疗结束后近期疗效有效和无效患者的临床特征比较,性别、年龄、组织分化程度、浸润深度的差异无统计学意义(P>0.05),VEGF、HIF-1α的表达以及淋巴结是否有转移的差异有统计学意义(P<0.05)。经多因素Logistic回归分析,VEGF、HIF-1α的阳性表达和淋巴结转移为影响食管鳞癌患者放疗疗效(P<0.05)的独立危险因素。结论食管鳞癌患者癌组织中VEGF、HIF-1α表达水平较高,且VEGF、HIF-1α的阳性表达与食管鳞癌患者近期放疗疗效密切相关。
Objective To explore the effect of expressions of vascular endothelial growth factor(VEGF),hypoxia inducible factor 1α(HIF-1α)in esophageal squamous cell carcinoma on radiotherapy in patients with esophageal squamous cell carcinoma.Methods 80 patients with esophageal squamous cell carcinoma treated with radiotherapy were collected.The pathological cancer tissue specimens were used as the observation group(n≤80)and the paracancerous tissue specimens as the control group(n≤80).The expressions of VEGF,HIF-1αin cancer tissues and paracancerous tissues were compared,and the correlations of expressions of VEGF,HIF-1αwere analyzed.The factors that may affect the short-term curative effect of radiotherapy in patients with esophageal squamous cell carcinoma were analyzed by univariate and multivariate analysis.Results The positive expression rate of VEGF,HIF-1αin the observation group was significantly higher than that of the control group(67.5%vs.7.5%,76.25%vs.11.25%,P<0.05).There was a positive correlation between the expression of HIF-1αand VEGF.There was no significant difference in gender,age,degree of tissue differentiation and depth of invasion between effective and ineffective patients after radiotherapy(P>0.05).There was significant difference in the expressions of HIF-1α,and VEGF,lymph node metastasis between effective and ineffective patients after radiotherapy(P<0.05).Multivariate Logistic regression analysis showed that the positive expressions of VEGF,HIF-1αand lymph node metastasis were independent risk factors for radiotherapy efficacy in patients with esophageal squamous cell carcinoma(P<0.05).Conclusion The expressions of VEGF,HIF-1αof cancer tissue in esophageal squamous cell carcinoma are high,and the positive expressions of VEGF,HIF-1αare closely related to the short-term radiotherapy effect in patients with esophageal squamous cell carcinoma.
