摘要
目的:评价布地奈德混合沙丁胺醇雾化吸入对单肺通气兔肺功能的影响。方法:健康雄性新西兰大白兔32只,5~6月龄,体重2.5~3.0 kg。采用随机数字表法分为4组(n=8):双肺通气组(TLV组)、单肺通气组(OLV组)、布地奈德雾化吸入组(B组)和布地奈德混合沙丁胺醇雾化吸入组(B+S组)。TLV组双肺通气3 h,余3组左侧单肺通气2 h,随后双肺通气1 h,B组于单肺通气前雾化吸入布地奈德1 mg(生理盐水稀释至2 ml),B+S组于单肺通气前雾化吸入沙丁胺醇0.15 mg/kg+布地奈德0.5 mg(生理盐水稀释至2 ml)。TLV组与OLV组均雾化吸入等量生理盐水。采用容量控制通气模式,于吸入前(T 0)、吸入后15 min(T 1)、吸入后1 h(T 2)和通气结束即刻(T 3)时采集股动脉血样行动脉血气分析,计算氧合指数,采集混合静脉血样测定相应指标,计算肺内分流率(Qs/Qt),记录气道峰压(P peak)、气道平台压(P plat)、气道阻力(Raw)和肺顺应性(C dyn)。结果:与TLV组比较,OLV组、B组和B+S组T 2,3时乳酸浓度升高,T 1-3时氧合指数和C dyn降低,Qs/Qt、Raw、P plat和P peak升高(P<0.05);与OLV组比较,B组和B+S组T 2,3时乳酸浓度降低,T 1-3时氧合指数和C dyn升高,Qs/Qt、Raw、P plat和P peak降低(P<0.05);与B组比较,B+S组T 1-3时C dyn升高,Qs/Qt、Raw和P peak降低(P<0.05)。结论:布地奈德混合沙丁胺醇雾化吸入可有效改善单肺通气兔肺功能。
Objective To evaluate the effect of inhaled aerosolized budesonide and salbutamol on lung function during one-lung ventilation(OLV)in rabbits.Methods Thirty-two healthy male New Zealand white rabbits,aged 5-6 months,weighing 2.5-3.0 kg,were randomized into 4 groups(n=8 each)using a random number table method:two-lung ventilation(TLV)group,OLV group,inhalation of aerosolized budesonide group(group B)and inhalation of aerosolized budesonide and salbutamol group(group B+S).Bilateral lungs were ventilated for 3 h in group TLV,the left lung was ventilated for 2 h followed by 1-h TLV in OLV,B and B+S groups,aerosolized budesonide 1 mg(diluted to 2 ml in normal saline)was inhaled before OLV in group B,and aerosolized salbutamol 0.15 mg/kg plus budesonide 0.5 mg was inhaled before OLV in group B+S.The equal volume of aerosolized normal saline was delivered in TLV and OLV groups.Volume-controlled ventilation was used in all groups.Arterial blood samples were obtained for blood gas analysis before aerosol inhalation(T0),15 min and 1 h after aerosol inhalation(T1,2),and at the end of ventilation(T3).Oxygenation index was calculated.Mixed venous blood samples were collected to determine the corresponding parameters.The pulmonary shunt(Qs/Qt)was calculated.Peak airway pressure(Ppeak),airway platform pressure(Pplat),airway resistance(Raw),and lung compliance(Cdyn)were continuously monitored and recorded at T0-T3.Results Compared with group TLV,the concentration of lactic acid was significantly increased at T2,3,oxygenation index and Cdyn were decreased at T1-3,and Qs/Qt,Raw,Pplat and Ppeak were increased in OLV,B and B+S groups(P<0.05).Compared with group OLV,the concentration of lactic acid was significantly decreased at T2,3,oxygenation index and Cdyn were increased at T1-3,and Qs/Qt,Raw,Pplat and Ppeak were decreased in B and B+S groups(P<0.05).Compared with group BD,the Cdyn was significantly increased at T1-3,and Qs/Qt,Raw and Ppeak were decreased in group B+S(P<0.05).Conclusion Inhaled aerosolized budesonide and salbutam
