急性脑梗死患者颅内微出血发生的危险因素分析 被引量:8

摘要 目的 观察急性脑梗死患者颅内微出血发生情况,并分析其影响因素.方法 选取2017年3月—2018年5月在海军安庆医院接受治疗的急性脑梗死患者为研究对象,统计患者颅内微出血发生情况,分析其影响因素,并探讨合并有高同型半胱氨酸血症的高血压与患者颅内微出血发生的关系.结果 180例急性脑梗死患者中发生颅内微出血者70例,颅内微出血发生率为38.89%,其中出血数目≥2处者25例;年龄≥65岁、有高血压、低TC、低TG、低谷丙转氨酶、高Hcy的急性脑梗死患者颅内微出血发生率较高,不同性别、高血压史、糖尿病史、FBG、肾功能水平的患者颅内微出血发生率无明显差别;将单因素分析有意义的因素作为自变量,将急性脑梗死患者发生颅内微出血作为因变量,进行回归分析.结果显示,TG、Hcy、高血压和年龄进入回归方程;将微出血的数目作为因变量,将单因素有意义的因素做为自变量,应用多重线性回归分析,选择逐步法,绘制标准化残差图的直方图和正态概率图,确定满足线性回归模型,得出:Hcy t=3.856,P=0.009;高血压t=3.267,P=0.013;年龄t=3.245,P=0.015.结论 急性脑梗死患者颅内微出血发生率较高,且受血脂和同型半胱氨酸水平以及高血压的影响. Objective To analyze the risk factors for cerebral micro-bleeds(CMBs)in patients with acute cerebral infarction.Methods The clinical data of 180 patients with acute cerebral infarction,100 males and 80 females,aged(55.98±4.67)were collected and analyzed by SPSS 11.5.Results CMBs were found in 70 patients(38.89%).Tlie pa-tients aged ≥65,with hypertension,and with low serum total cholesterol(TC),low triglyceride(TG),low alanine amino transferase(ATT),and high homocystinuria(Hey)showed higher CMB rates(all P<0.05).Regression analysis showed that TG,Hey,hypertension,and age entered the regressiofi equation.The incidence of intracranial microbleeds in patients with cerebral infarction was not related with gender,history of hypertension,history of diabetes,fasting blood sugar,and renal function.Using the meaningful factors as independent variables,and GMBs in patients with acute cerebral infarction as dependent variables,the univariate analysis showed that TG,Hey,hypertension and age entered the regression equation.Using the number of CMBs as the dependent variable and the meaningful single factors as the independent variables,the multiple linear regression analysis selected the stepwise method to draw the histogram of standardized residual and normal probability plot of the graph so as to satisfy the linear regression model,and the following results were obtained:Hcy t=3.856,P=0.009;hypertension t=3.267,P=0.013;and age t=3.245,P=0.015.Conclusion The incidence of intracranial microbleeding in patients with acute cerebral infarction is high and is affected by blood lipid levels and homocysteine level.
