摘要
目的观察巩膜潜池式小梁切除联合新鲜羊膜移植术在闭角型青光眼治疗中的效果,为闭角型青光眼手术治疗提供临床依据。方法选取符合纳入标准的闭角型青光眼患者106例(128只眼),按随机数字表法将其分为治疗组53例(64只眼)和对照组53例(64只眼),治疗组予巩膜潜池式小梁切除联合新鲜羊膜移植术,对照组予常规小梁切除术。观察术前、术后3个月和术后6个月2组的最佳矫正视力(BCVA)、眼压情况及滤过泡形态。结果 (1)BCVA:治疗后3个月,2组比较差异有统计学意义(t=3.333,P=0.002)。治疗前后比较,治疗组有统计学意义(t=5.517,P=0.000),对照组无统计学意义(t=1.111,P=0.298)。治疗后6个月,2组比较差异均有统计学意义(t=2.258,P=0.023)。治疗前后比较,治疗组(t=7.931,P=0.000)和对照组(t=3.871,P=0.000),差异均有统计学意义。(2)眼压:治疗后3个月,2组比较差异有统计学意义(t=9.184,P=0.000)。治疗前后比较,治疗组(t=40.281,P=0.000)和对照组(t=15.718,P=0.000)均有统计学意义。治疗后6个月,2组比较差异有统计学意义(t=5.517,P=0.000)。治疗前后比较,治疗组(t=39.494,P=0.000)和对照组(t=18.804,P=0.000),差异均有统计学意义。(3)滤过泡形态:治疗后3个月,2组比较差异无统计学意义(x^2=2.712,P=0.092)。治疗后6个月,2组比较差异有统计学意义(x^2=4.341,P=0.041)。结论巩膜潜池式小梁切除联合新鲜羊膜移植手术,术后可更好地抑制滤过道瘢痕化,减少手术后浅前房、低眼压的发生,为闭角型青光眼的治疗提供新的治疗方法。
OBJECTIVE To explore the effect of scleral concave pool trabeculectomy combined with fresh amniotic membrane transplantation in the treatment of angle-closure glaucoma,and provide clinical basis for the surgical treatment of angle-closure glaucoma. METHODS A total of 106 patients(128 eyes) with angle-closure glaucoma meeting the inclusion criteria were selected and randomly divided into the treatment group with 53 cases(64 eyes), the control group with 53 cases(64 eyes).The treatment group received scleral concave trabeculectomy combined with fresh amniotic membrane transplantation, while the control group received common trabeculectomy. Six months after treatment, the difference between two groups was of statistical significance(t=2.258, P=0.023).
