摘要
目的评价生物羊膜在青光眼小梁切除术中的作用。方法收集2010年1月至2011年6月就诊我院诊断为青光眼患者72例(98只眼),按随机数字表分为两组:小梁切除术组和小梁切除联合羊膜移植术组,由同一位医师完成手术,术前和术后1周,1、3、12及18个月随访观察眼压;18个月时采用超声生物显微镜(UBM)测量滤过泡长径和宽径的长度,计算手术成功率。结果小梁切除联合羊膜移植术组术后眼压的波动较小梁切除术组低。UBM检测发现18个月后小梁切除术组滤过泡长径为(3.81±0.91)mm,小梁切除联合羊膜移植术组滤过泡长径为(7.11±2.08)mm,两者间差异具有统计学意义(t=8.708,P<0.001);小梁切除术组滤过泡宽径为(3.14±0.96)mm,小梁切除联合羊膜移植术组滤过泡宽径为(4.78±1.20)mm,两者间差异具有统计学意义(t=6.324,P<0.001)。18个月时小梁切除术组手术完全成功率为56.8%(21只眼),小梁切除联合羊膜移植术组手术完全成功率为84.4%(27只眼),两者间差异具有统计学意义(χ2=6.182,P=0.013);18个月时小梁切除术组手术条件成功率为75.7%(28只眼),小梁切除联合羊膜移植术组手术条件功率为93.8%(30只眼),两者间差异具有统计学意义(χ2=4.183,P=0.041)。结论小梁切除联合羊膜移植术可明显减轻滤过道瘢痕化,形成功能性滤过泡,提高手术成功率,是青光眼治疗一种安全有效的手术方法。
Objective To investigate the effect of biological amnion in trabeculectomy.Method 72 glaucoma patients（98 eyes） were collected from January 2010 to June 2011 in our hospital.Patients were divided into two groups： trabeculectomy group and trabeculectomy combined with amniotic membrane graft group according to the random number table.Both surgical methods were performed by the same physician.Intraocular pressure（IOP） was measured preoperative and 1 week,1 month,3 months,12 months and 18 months post-operation follow-up.18 months post-operation,the bleb long diameter and the bleb wide diameter were measured using ultrasound biomicroscopy（UBM） and surgery success rate was calculated.Results Postoperative IOP fluctuation of trabeculectomy combined with amniotic membrane graft group was smaller than that of trabeculectomy group.18 months post-operation,UBM showed that the bleb long diameter of trabeculectomy group was（3.81 ± 0.91） mm and that of trabeculectomy combined with amniotic membrane graft group was（7.11 ± 2.08） mm.The difference was statistical significances（t = 8.708,P 0.001）.The bleb wide diameter of trabeculectomy group was（3.14 ± 0.96） mm and that of trabeculectomy combined with amniotic membrane graft group was（4.78 ±1.20） mm.The difference was statistical significances（t =6.324,P 0.001）.The complete surgical success rate of trabeculectomy group was 56.8%（21 eyes） and that of trabeculectomy combined with amniotic membrane graft group was 84.4%（27 eyes）.The difference was statistical significances（χ2 = 6.182,P = 0.013）.The conditional surgical success rate of trabeculectomy group was 75.7%（28 eyes） and that of trabeculectomy combined with amniotic membrane graft group was 93.8%（30 eyes）.The difference was statistical significances（χ2 = 6.182,P = 0.013）.Conclusion Trabeculectomy combined with amniotic membrane graft can significantly relieve the scarring in filtration canal,facilitate the formation of functional blebs,and improve the surgery
