摘要
目的:探讨超声引导下经皮活检在乳腺癌淋巴结转移诊断中的应用及对穿刺成功率的影响.方法:选取2017年1月-2019年10月于本院接受手术治疗的69例乳腺疾病患者的临床资料进行回顾性分析.所有患者行超声引导下经皮活检.比较超声引导下经皮活检与手术病理诊断的诊断结果,分析超声引导下经皮活检穿刺活检成功率,观察并发症发生情况.结果:69例乳腺癌患者中,浸润性导管癌31例(44.93%)、浸润性小叶癌12例(17.39%)、浸润性乳头状癌11例(15.94%)、导管原位癌伴微浸润15例(21.74%);淋巴转移61例(88.41%)、淋巴未转移8例(11.59%).超声引导下经皮活检诊断结果为淋巴结转移53例,淋巴结未转移16例,诊断准确率为79.71%(55/69),与病理诊断结果的Kappa值为0.901.穿刺活检成功率100%.有7例患者出现穿刺部位轻微疼痛,无其他不良反应发生.结论:应用超声引导下经皮活检诊断乳腺癌淋巴结转移结果确切,穿刺活检成功率高,可为临床对乳腺癌的病情诊断、治疗方案及预后提供重要的参考价值,值得临床推广应用.
Objective:To investigate the application of ultrasound-guided percutaneous biopsy in the diagnosis of breast cancer lymph node metastasis and its effect on the success rate of puncture.Method:The clinical data of 69 patients with breast disease treated with surgery in our hospital from January 2017 to October 2019 were retrospectively analyzed.The results of ultrasound-guided percutaneous biopsy and surgical pathological diagnosis were compared.The success rate of ultrasound-guided percutaneous puncture biopsy was analyzed,and the incidence of complications was observed.Result:Among the 69 breast cancer patients,31 cases(44.93%)were infiltrating ductal carcinoma,12 cases(17.39%)were infiltrating lobular carcinoma,11 cases(15.94%)were infiltrating papillary carcinoma,and 15 cases(21.74%)were ductal carcinoma in situ with microinfiltrating carcinoma.There were 61 cases(88.41%)with lymphatic metastasis and 8 cases(11.59%)with no lymphatic metastasis.Ultrasound guided percutaneous biopsy revealed lymph node metastasis in 53 cases and no lymph node metastasis in 16 cases,with a diagnostic accuracy rate of 79.71%(55/69),and a Kappa value of 0.901 compared with pathological diagnosis.The success rate of puncture biopsy was 100%.There were 7 patients with mild pain at the puncture site and no other adverse reaction occurred.Conclusion:The application of ultrasound-guided percutaneous biopsy in the diagnosis of lymph node metastasis of breast cancer has accurate results and high success rate of puncture biopsy,which can provide important reference value for clinical diagnosis,treatment plan and prognosis of breast cancer and is worthy of clinical promotion and application.
