乳腺癌腋窝淋巴结转移超声造影特征分析 被引量:18

Correlation between contrast-enhanced ultrasound features and axillary lymph node metastasis in female breast cancer

摘要 目的乳腺癌腋窝淋巴结是否转移被临床作为选择治疗方法及预后评估的重要参考依据。灰阶超声造影(contrast-enhanced ultrasound,CEUS)是一种对乳腺癌具有高准确率且无创的检查方法。本研究旨在探讨CEUS特征与乳腺癌腋窝淋巴结转移的关系。方法选取2013-01-01-2018-05-31儋州市人民医院收治的428例女性乳腺癌患者(772枚腋窝淋巴结)进行CEUS检查,其中术后病理确诊为转移的腋窝淋巴结560枚,非转移腋窝淋巴结212枚。采用单因素分析及Logistic逐步回归分析CEUS特征与乳腺癌腋窝淋巴结转移的相关性。结果单因素分析显示,周边声晕、血流分级、灌注顺序、周边汇聚、增强方式、边缘增强、增强后范围扩大、增强顺序、增强幅度、纵横比、最大皮质厚度、峰值强度、达峰时间及曲线下面积与乳腺癌腋窝淋巴结转移相关,差异有统计学意义,P<0.05。多因素分析显示,增强方式(OR=3.805,95%CI为2.503~4.972)、增强后范围扩大(OR=4.106,95%CI为2.845~5.746)、增强幅度(OR=3.868,95%CI为2.517~5.263)、最大皮质厚度(OR=3.154,95%CI为2.208~4.525)、峰值强度(OR=2.117,95%CI为1.805~3.214)和达峰时间(OR=1.824,95%CI为1.526~2.860)与乳腺癌腋窝淋巴结转移有关。结论 CEUS特征与乳腺癌腋窝淋巴结转移有关,对乳腺癌诊疗有一定的指导作用。 OBJECTIVE Whether or not axillary lymph node metastasis of breast cancer is clinically regarded as an important reference for the selection of treatment methods and prognosis evaluation.Contrast-enhanced ultrasound (contrast-enhanced ultrasound,CEUS) is a high accuracy and non-invasive method for breast cancer detection.The purpose of this study was to investigate the relationship between CEUS features and axillary lymph node metastasis of breast cancer.METHODS Totally 428 female breast cancer patients (772 axillary lymph nodes) admitted to Danzhou People's Hospital from January 2013 to May 2018 were examined by CEUS.Among them,560 axillary lymph nodes and 212 non-metastatic axillary lymph nodes were confirmed by pathology after operation.Correlation between Contrast-enhanced ultrasonography and axillary lymph node metastasis in Breast Cancer by Single Factor Analysis and Logistic Stepwise Regression.RESULTS Uni variate analysis showed that peripheral halo, blood flow grading,perfusion sequence,peripheral convergence, enhancement mode,edge enhancement,enlarged range after enhancement,enhancement sequence,enhancement amplitude, aspect ratio,maximum cortical thickness,peak intensity,peak time and area under curve in axillary lymph node metastasis were associated with breast cancer,the difference was statistically significant (P<0.05).Multivariate analysis showed enhancement mode (OR=3.805,95 % CI : 2.503 - 4.972), enlarged range (OR=4.106,95 % CI : 2.845 - 5.746), enhancement amplitude (OR=3.868,95 %CI : 2.517 - 5.263), maximum cortical thickness (OR=3.154,95 % CI : 2.208 - 4.525), peak intensity (OR=2.117,95%CI: 1.805 - 3.214) and peak time (OR=1.824,95%CI: 1.526 - 2.860) were associated with axillary lymph node metastasis in breast cancer.CONCLUSION Contrast-enhanced ultrasonography is useful in diagnosis of axillary lymph node metastasis of breast cancer.
