摘要
目的:探讨彩色多普勒超声(下文简称超声)对乳腺癌前哨淋巴结转移(SLN)的临床诊断价值。方法:选取本院2018年1月—2019年12月收治的58例乳腺癌患者作为研究对象,均进行术前超声及SLN活检术检查,同时在术后进行病理组织切片检测。分析58例患者的淋巴结转移情况,并总结SLN转移的发生情况及超声影像学特征,评价超声在SLN诊断中的价值。结果:以病理组织学检测结果为金标准,其结果显示58例患者中46例患者SLN正常,12例患者存在SLN转移,转移的患者中7例存在SLN微转移,5例存在SLN宏转移。SLN转移患者的长径、短径、峰值流速(Vmax)、阻力指数(RI)指标水平大于SLN正常患者,差异有统计学意义(P<0.05)。影像学特征显示SLN转移患者内部回声以不均匀为主,血流分布呈现混合型,SLN形态表现为椭圆形。超声诊断SLN转移的诊断符合率91.38%(53/58)。结论:超声在乳腺癌SLN转移筛查中具有较高的应用价值,对于是否开展腋窝淋巴结清扫术有重要的指导作用。
Objective To explore the clinical diagnostic value of color Doppler ultrasound(hereinafter referred to as ultrasound)for sentinel lymph node metastasis(SLN)in breast cancer.Methods A total of 58 breast cancer patients admitted to our hospital from January 2018 to December 2019 were selected as the research objects.All of them were examined by preoperative ultrasound and SLN biopsy,and pathological tissue biopsies were examined after surgery.Analyze 58 patients with lymph node metastasis,and summarize the occurrence of SLN metastasis and ultrasound imaging characteristics,and evaluate the value of ultrasound in the diagnosis of SLN.Results Using the histopathological test results as the gold standard,the results showed that 46 of 58 patients had normal SLN,12 patients had SLN metastasis,7 of the metastatic patients had SLN micrometastasis,and 5 had SLN macrometastasis.The long diameter,short diameter,peak velocity(Vmax)and resistance index(RI)of patients with SLN metastasis were higher than those with normal SLN,and the difference was statistically significant(P<0.05).The imaging features show that the internal echo of patients with SLN metastasis is mainly uneven,the blood flow distribution is mixed,and the shape of SLN is oval.The diagnostic coincidence rate of ultrasound diagnosis of SLN metastasis was 91.38%(53/58).Conclusion Ultrasound has high application value in the screening of SLN metastasis of breast cancer,and has an important guiding role in whether to carry out axillary lymph node dissection.
