摘要
本文旨在探讨白细胞计数(WBC)、血清淀粉样蛋白A(SAA)、C-反应蛋白(CRP)检测在儿童感染性肺炎治疗中的价值。选取2019年7月-9月在肇庆市端州区妇幼保健院儿科就诊的儿童183例,其中细菌感染组54例,病毒感染组49例,肺炎支原体感染组37例,对照组43例,测定WBC、SAA及CRP单一检测及联合检测的诊断结果,并分别作ROC曲线分析。细菌组、病毒组和肺炎支原体组的WBC、SAA和CRP均高于对照组,细菌组的CRP显著高于病毒组和肺炎支原体组,病毒组的SAA显著低于细菌组和肺炎支原体组,比较存在统计学差异(P<0.05)。WBC、SAA及CRP联合检测的灵敏度、特异度均显著高于三者的单一检测(P<0.05)。此外,三指标联合诊断对于细菌和病毒感染的诊断效能显著高于肺炎支原体。在这三个指标中,SAA的升高幅度最为明显,曲线下面积在细菌、病毒和肺炎支原体中分别是0.737、0.698和0.682,灵敏度和特异度也分别高于其他两个指标。WBC、SAA及CRP联合检测可以有效鉴别诊断由细菌、病毒或者肺炎支原体导致的儿童感染性肺炎,诊断精确率更优,为临床治疗提供有力的参考依据。
The purpose of this study was to investigate the value of white blood cells(WBC),serum amyloid A(SAA)and Creactive protein(CRP)in the treatment of child infectious pneumonia.183 cases of children admitted to the pediatric department of maternal and child health hospital in duanzhou district of zhaoqing city from July to September 2019 were selected,including 54 cases of bacterial infection group,49 cases of viral infection,37 casesof mycoplasma pneumoniae infection and 43 cases as control group.The single and combined diagnostic results of WBC,SAA and CRP were compared comprehensively,and then ROC curve analysis was made respectively.The WBC,SAA and CRP of the bacterial group,viral group and mycoplasma pneumoniae group were all higher than those of the control group,the CRP of the bacterial group was significantly higher than that of the viral group and mycoplasma pneumoniae group,and the SAA of the viral group was significantly lower than that of the bacterial group and mycoplasma pneumoniae group with statistical difference(P<0.05).The sensitivity and specificity of the combined detection of WBC,SAA and CRP were significantly higher than that of the single detection(P<0.05).In addition,the diagnostic efficacy of the three indicators for bacterial and viral infection was significantly higher than that of Mycoplasma pneumoniae.In the three indicators,the increase of SAA was the most obvious,the area under the curve was 0.737,0.698 and 0.682 in bacteria,virus and Mycoplasma pneumoniaerespectively,and the sensitivity and specificity were also higher than the other two indicators.Combined detection of WBC,SAA and CRP can effectively differentiate and diagnose child infectious pneumonia caused by bacterial,virus and mycoplasma pneumoniae,with better diagnostic accuracy,providing a strong reference for clinical treatment.
